• All Dietitian services are to be conducted at the GreenLeaf Nutrition & Dietetics Center premises located on Bole Road, Addis Ababa, unless otherwise agreed in writing for home-visit or telehealth arrangements.
• Payment is due within fourteen (14) days of the client's written acceptance of this Quotation Estimate. Payment may be made via bank transfer to the account of GreenLeaf Nutrition & Dietetics Center at Commercial Bank of Ethiopia, Addis Ababa branch, or in person at the center's front desk.
• A 50% advance payment is required to confirm the initial Dietitian consultation appointment. The remaining balance is due upon completion of the final follow-up session.
• Cancellations must be communicated at least 48 hours in advance. Late cancellations or no-shows will incur a 25% fee of the scheduled session rate.
• All dietary plans and nutritional assessments are confidential and protected under the Ethiopian Data Protection Proclamation No. 1104/2019. The Dietitian shall not disclose client information to third parties without written consent.
• This Quotation Estimate does not constitute a medical diagnosis. The Dietitian operates under the supervision of a licensed physician where clinical nutrition therapy is indicated. Clients with complex medical conditions should coordinate care with their primary physician in Addis Ababa.
• Any additional services beyond the scope described in this document will be quoted separately and require written approval before commencement.
• This Quotation Estimate is governed by the laws of the Federal Democratic Republic of Ethiopia. Any disputes arising from the services described herein shall be resolved through the Addis Ababa City Courts.
