This Quotation Estimate is issued by NutriLife Jakarta Wellness Center to provide a comprehensive and itemized cost breakdown for the engagement of a licensed and certified Dietitian to deliver nutritional assessment, meal planning, and ongoing dietary management services for the employees of PT. Nusantara Health & Wellness Group. All Dietitian services described herein shall be performed in accordance with the standards set by the Indonesian Association of Dietitians (PDI - Perhimpunan Dietisien Indonesia) and the regulatory requirements of the Ministry of Health of the Republic of Indonesia. The Dietitian assigned to this engagement holds a minimum of five (5) years of clinical experience in Indonesia Jakarta and is registered with the Indonesian Dietitian Council (IDAI-affiliated).

- This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. After the expiry date, all prices listed herein are subject to revision based on prevailing market rates in Indonesia Jakarta and changes in the Indonesian government's Value Added Tax (PPN) regulations.
- All Dietitian services shall be performed by qualified professionals registered with the Perhimpunan Dietisien Indonesia (PDI). The lead Dietitian shall hold a minimum of a Bachelor's degree in Nutrition (S1 Gizi) from an accredited Indonesian university and a valid professional practice license (STR - Surat Tanda Registrasi).
- Payment terms: 40% advance payment upon acceptance of this Quotation Estimate, 30% upon completion of the first quarter of services, and 30% upon final delivery of the twelfth monthly report. All payments shall be made via bank transfer to the account of NutriLife Jakarta Wellness Center, Bank Mandiri, Account No. 137-00-2245678-9, Jakarta Branch.
- The client agrees to provide a suitable consultation space at their Indonesia Jakarta premises for in-person Dietitian sessions, including access to basic medical records (with employee consent) and canteen kitchen areas for menu audits.
- All meal plans and nutritional recommendations provided by the Dietitian shall strictly adhere to halal dietary requirements in accordance with the Indonesian Halal Product Assurance Agency (BPJPH) guidelines.
- Confidentiality: All employee health data, dietary records, and consultation notes handled by the Dietitian team shall be treated as strictly confidential in compliance with Indonesian Law No. 36 of 2009 on Health and Law No. 27 of 2022 on Personal Data Protection (UU PDP).
- Any additional Dietitian services beyond the scope defined in this Quotation Estimate shall be subject to a separate written amendment and additional cost approval by both parties.
- This Quotation Estimate does not constitute a binding contract until a formal Service Agreement is executed by both parties. The terms herein serve solely as a cost estimation and service scope reference.
- Force majeure events, including but not limited to natural disasters, government-mandated lockdowns in Indonesia Jakarta, or public health emergencies, shall suspend service delivery obligations without penalty to either party.
By signing below, the client acknowledges receipt of this Quotation Estimate for Dietitian services and confirms that the scope, pricing, and terms outlined above are acceptable. This signature does not constitute a final contract but serves as a formal expression of intent to proceed with the engagement of the Dietitian team from NutriLife Jakarta Wellness Center.
