3.1 Validity: This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. After the expiry date, the Dietitian service rates in this Quotation Estimate may be subject to revision based on prevailing market conditions in Iraq Baghdad.
3.2 Payment Terms: A 30% advance payment (1,950,165 IQD) is due upon acceptance of this Quotation Estimate. The remaining balance shall be paid in three equal quarterly installments. All payments are to be made in Iraqi Dinar via bank transfer to the account of Al-Rafidain Nutrition & Dietetics Center, Bank of Baghdad, Al-Mansour Branch.
3.3 Cancellation Policy: Cancellations of scheduled Dietitian appointments must be made at least 24 hours in advance. Late cancellations or no-shows will incur a 50% fee of the scheduled Dietitian session rate. The client may terminate the 12-month program with 30 days' written notice; no refund will be issued for services already rendered.
3.4 Confidentiality: All patient data, dietary records, and medical information handled by our Dietitian team in Iraq Baghdad are protected under the Iraqi Personal Data Protection Law (2023) and the professional ethical code of the Iraqi Dietitians Association.
3.5 Scope Limitation: This Quotation Estimate covers Dietitian and nutritional counseling services only. It does not include medical diagnosis, pharmaceutical prescriptions, or laboratory testing. Any required blood work or medical referrals will be billed separately by the relevant healthcare provider in Iraq Baghdad.
3.6 Force Majeure: In the event of circumstances beyond reasonable control (including but not limited to public health emergencies, civil unrest, or government-mandated closures in Iraq Baghdad), the Dietitian services may be temporarily suspended or transitioned to telehealth delivery without penalty to either party.
3.7 Governing Law: This Quotation Estimate and the resulting service agreement shall be governed by and interpreted in accordance with the laws of the Republic of Iraq. Any disputes shall be resolved through the competent courts in Baghdad, Iraq.
Note: All Dietitian services referenced in this Quotation Estimate are provided by a licensed Dietitian holding a valid practice permit issued by the Iraqi Ministry of Health. The Dietitian is a member in good standing of the Iraqi Dietitians Association and the International Dietitian Federation (IDF). The client is encouraged to verify credentials through the official Iraqi Ministry of Health registry prior to acceptance of this Quotation Estimate.By signing below, the client acknowledges receipt of this Quotation Estimate for Dietitian services in Iraq Baghdad and agrees to the terms, conditions, and pricing outlined herein. This signature constitutes formal acceptance of the Quotation Estimate and authorizes Al-Rafidain Nutrition & Dietetics Center to commence the 12-month Dietitian program as described.
