This Quotation Estimate is issued by NutriLife Wellness Centre Sdn. Bhd. to provide a comprehensive breakdown of professional Dietitian services to be delivered within the Malaysia Kuala Lumpur metropolitan area. Our registered Dietitian team, all of whom are certified by the Malaysian Dietitians Association (MDA) and the Malaysian Medical Council, will provide personalised nutritional assessment, meal planning, and ongoing dietary management tailored to the specific health objectives of the client. All services referenced in this Quotation Estimate are to be conducted at our Malaysia Kuala Lumpur clinic premises or, where applicable, at the client's designated corporate office within the Klang Valley region.

- Validity: This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. Any acceptance beyond this period is subject to re-evaluation of rates by NutriLife Wellness Centre Sdn. Bhd.
- Payment Terms: A 50% deposit (RM 3,068.70) is required upon acceptance of this Quotation Estimate to secure the Dietitian's schedule. The remaining 50% balance (RM 3,068.70) is due within fourteen (14) days of programme completion.
- Cancellation Policy: Cancellation of any scheduled Dietitian session must be communicated at least 48 hours in advance. Late cancellations or no-shows will be charged at 75% of the applicable session fee.
- Scope Limitation: The Dietitian services outlined in this Quotation Estimate are strictly for nutritional counselling, dietary planning, and health education purposes. They do not constitute medical diagnosis or treatment. Any underlying medical conditions should be managed by a registered medical practitioner in Malaysia Kuala Lumpur.
- Confidentiality: All client data, dietary records, and health information handled by our Dietitian team are protected under the Personal Data Protection Act 2010 (PDPA) of Malaysia. No information will be disclosed to third parties without written consent.
- Service Location: Unless otherwise stated, all in-person Dietitian consultations will take place at our registered Malaysia Kuala Lumpur clinic address. Telehealth sessions will be conducted via a secure, encrypted video platform.
- Revisions: Up to two (2) revisions of the personalised meal plan are included. Additional revisions will be billed at RM 150.00 per revision.
- Governing Law: This Quotation Estimate and any resulting agreement shall be governed by and construed in accordance with the laws of Malaysia. Any disputes shall be subject to the exclusive jurisdiction of the courts in Malaysia Kuala Lumpur.
By signing below, the client acknowledges and accepts all terms, conditions, and pricing outlined in this Quotation Estimate for the Dietitian services to be provided in Malaysia Kuala Lumpur. The client agrees to the payment schedule and understands that the commencement of services is contingent upon receipt of the initial deposit.
