3.1 This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. All prices are quoted in New Zealand Dollars (NZD) and include Goods and Services Tax (GST) at the current New Zealand rate of 15%.
3.2 A non-refundable deposit of 50% ($845.00) is required to secure the client's appointment schedule with the Dietitian. The remaining balance is due within fourteen (14) days of the final consultation session.
3.3 Cancellations must be provided with a minimum of 48 hours' notice. Late cancellations or no-shows will incur a fee of 50% of the scheduled session rate, as is standard practice for Dietitian services in New Zealand Auckland.
3.4 All services are delivered by a Dietitian registered with Dietitians New Zealand (DNZ) and are subject to the DNZ Code of Ethics. The Dietitian will not provide medical diagnoses; all recommendations are nutritional in nature and complement, not replace, the client's existing medical treatment plan.
3.5 This Quotation Estimate does not include the cost of laboratory blood tests, which are to be arranged and paid for separately through the client's General Practitioner or a private pathology provider in Auckland.
3.6 The Dietitian reserves the right to adjust the programme scope if the client's clinical condition changes materially during the engagement. Any additional sessions beyond those itemised above will be billed at the standard Auckland Dietitian consultation rate of $195.00 per 60-minute session.
3.7 All client health information is handled in strict accordance with the New Zealand Privacy Act 2020 and the Health Information Privacy Code 1994. Records are stored securely at the Auckland Central practice and are retained for a minimum of seven (7) years as required by New Zealand health record-keeping standards.
By signing below, the client acknowledges that this Quotation Estimate for Dietitian services in New Zealand Auckland has been reviewed and accepted in full. The client agrees to the terms, conditions, and payment schedule outlined in this document.
