4.1 Payment for this Quotation Estimate is due within 14 days of acceptance. Payment may be made via bank transfer to Wellington Nutrition & Dietetics Centre, Account Name: Wellington Nutrition Ltd, Bank: ANZ New Zealand, Account Number: 01-0456-7890123-00, or by EFTPOS at our Te Aro office in Wellington.
4.2 A 50% deposit is required to secure your initial Dietitian appointment. The remaining balance is due at the conclusion of the three-month programme.
4.3 Cancellations made less than 48 hours before a scheduled Dietitian session will incur a fee of 50% of the session rate.
4.4 All prices in this Quotation Estimate are in New Zealand Dollars (NZD) and include Goods and Services Tax (GST) at 15% as per New Zealand tax law.
5.1 This Quotation Estimate is a formal offer by Wellington Nutrition & Dietetics Centre and does not constitute a binding contract until signed and returned by the client.
5.2 Our registered Dietitian will operate in accordance with the Code of Ethics of Dietitians New Zealand and all applicable New Zealand health and privacy legislation, including the Privacy Act 2020.
5.3 All dietary advice provided by our Dietitian in New Zealand Wellington is for general nutritional guidance and does not replace medical treatment. For acute medical conditions, our Dietitian will coordinate care with your treating physician.
5.4 Client records and dietary plans are stored securely in compliance with New Zealand data protection regulations and are retained for a period of seven years.
5.5 This Quotation Estimate may be withdrawn if the client fails to make payment within the stipulated timeframe.
