Validity: This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. Prices are subject to revision upon expiry due to inflationary adjustments in the Nigeria Abuja market.
Payment Terms: A non-refundable deposit of 50% (₦147,812.50) is required to secure the Dietitian's schedule and confirm the engagement. The remaining 50% balance is due upon completion of the final follow-up session and delivery of the progress report.
Payment Method: Payments may be made via bank transfer to GreenPlate Nutrition & Wellness Centre (Access Bank, Account No. 0123456789, Branch: Wuse II, Abuja), or by certified cheque payable to the company name. Mobile money (OPay, PalmPay) is accepted for amounts up to ₦50,000.
Cancellation Policy: Cancellations made more than 72 hours before a scheduled Dietitian session will incur no penalty. Cancellations within 72 hours will be charged at 50% of the session fee. No-shows will be charged in full.
Confidentiality: All client health information, dietary records, and medical data shared during Dietitian consultations are protected under the National Health Act 2014 and the Data Protection Regulation (DPR) 2019 of Nigeria. No information will be disclosed to third parties without written consent.
Scope Limitation: The Dietitian services provided under this Quotation Estimate are for nutritional counselling and dietary management only. They do not constitute medical diagnosis, prescription of pharmaceutical drugs, or surgical intervention. Clients with acute medical conditions should continue to see their primary physician in parallel.
Revisions: Up to two (2) revisions of the personalised diet plan are included at no additional cost. Further revisions or changes in scope will be quoted separately as a supplementary Quotation Estimate.
Governing Law: This Quotation Estimate and any resulting agreement shall be governed by the laws of the Federal Republic of Nigeria, with jurisdiction in the courts of Abuja, FCT.
