Payment for the services detailed in this Quotation Estimate shall be made in Philippine Pesos (PHP) via bank transfer to the Manila Wellness Dietetics Center account at BDO Unibank, Makati Branch, Philippines Manila, or through accredited electronic payment channels. A 50% down payment is required upon acceptance of this Quotation Estimate to secure the scheduled Dietitian appointment slots. The remaining balance shall be settled within fifteen (15) calendar days from the completion of the service package. Late payments shall incur a 1.5% monthly interest charge as per Philippine commercial practice.
Important Conditions of This Quotation Estimate
- This Quotation Estimate is valid for thirty (30) days from the date of issue. After the expiration date, pricing for Dietitian services may be subject to revision based on market conditions in Philippines Manila.
- All Dietitian consultations are conducted by PRC-licensed Registered Dietitians with a minimum of five (5) years of clinical experience in the Philippines Manila healthcare setting.
- Cancellations or rescheduling of Dietitian appointments must be made at least forty-eight (48) hours in advance. Cancellations within 48 hours will be subject to a 50% fee of the scheduled session.
- This Quotation Estimate does not constitute a medical diagnosis. The Dietitian services provided are for nutritional counseling and dietary management only. Clients with acute medical conditions should consult their primary physician in conjunction with the Dietitian program.
- All personal health information shared during Dietitian consultations in Philippines Manila is protected under the Philippine Data Privacy Act of 2012 (Republic Act No. 10173).
- Home visit services are limited to the Metro Manila / Philippines Manila area (Makati, Mandaluyong, Pasig, Quezon City, Taguig, Marikina, and surrounding municipalities). Travel beyond this radius will incur an additional surcharge of PHP 50 per kilometer.
- Corporate workshop rates quoted in this Quotation Estimate are for groups of up to fifty (50) participants. Additional attendees beyond this number will be charged at PHP 250 per person.
- This Quotation Estimate is non-transferable and is issued specifically for the client named above. Any change in client or scope of Dietitian services requires a revised Quotation Estimate.
By signing below, the client acknowledges receipt of this Quotation Estimate for Dietitian services in Philippines Manila and agrees to the terms, conditions, and pricing outlined herein. The signed Quotation Estimate shall serve as the binding agreement between Manila Wellness Dietetics Center and the client for the delivery of the specified nutritional services.
