2. Scope of Dietitian ServicesOur qualified Dietitian, Ms. Lim Serene (HSA-registered, Singapore Singapore), will provide the following personalised nutrition and dietary counselling services as detailed in this Quotation Estimate:

- This Quotation Estimate is valid for a period of thirty (30) calendar days from the date of issue. Any acceptance beyond this period may be subject to revised pricing by the Dietitian practice.
- All Dietitian services outlined in this Quotation Estimate shall be conducted at our premises located in Singapore Singapore, unless otherwise agreed in writing for telehealth or home-visit arrangements.
- Payment is due within fourteen (14) days from the date of acceptance of this Quotation Estimate. Payment may be made via bank transfer, PayNow, or credit card. A 2% surcharge applies to credit card transactions.
- A 50% deposit is required to secure the initial Dietitian consultation appointment. The remaining balance shall be payable upon completion of the full service package.
- Cancellations must be made at least forty-eight (48) hours in advance. Late cancellations or no-shows will incur a 50% fee of the scheduled Dietitian session rate.
- All dietary plans, meal programmes, and nutritional reports prepared by our Dietitian are the intellectual property of NutriLife Dietetics Centre Pte Ltd and are intended solely for the client named in this Quotation Estimate.
- This Quotation Estimate does not constitute a medical diagnosis or treatment plan. The Dietitian services provided are complementary to, and not a substitute for, medical care provided by a licensed physician in Singapore Singapore.
- Client health data and dietary information shared during Dietitian consultations are protected under the Personal Data Protection Act (PDPA) of Singapore Singapore and shall not be disclosed to third parties without written consent.
- Any disputes arising from this Quotation Estimate shall be governed by the laws of Singapore Singapore and subject to the exclusive jurisdiction of the courts of Singapore Singapore.
- This Quotation Estimate is non-transferable and is issued exclusively to the client named above. Any change in client details requires a revised Quotation Estimate from our office.
By signing below, the client acknowledges receipt of this Quotation Estimate and agrees to the terms and conditions set forth herein for the engagement of Dietitian services in Singapore Singapore.
