1. This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. Upon expiry, the Dietitian reserves the right to revise rates in line with current South African market conditions and the SADA fee schedule.
2. Payment is due within fourteen (14) days of invoice date. Invoices will be issued monthly in arrears. Late payments will attract interest at the rate of 1.5% per month as per the National Credit Act of South Africa.
3. A 30% deposit is required to secure the Dietitian's schedule and commence the initial nutritional screening phase in Cape Town.
4. All services are subject to the client providing a suitable, private consultation space at the Woodstock office. The Dietitian will bring all necessary assessment tools, dietary reference materials, and South African food composition data.
5. Cancellations of scheduled Dietitian sessions must be communicated at least 48 hours in advance. No-fee rescheduling is permitted once per quarter.
6. This Quotation Estimate does not include the cost of medical investigations (blood work, biopsies, etc.) which must be arranged separately through a registered medical practitioner in South Africa.
7. All client data and nutritional records are protected in accordance with the Protection of Personal Information Act (POPIA), 2013, of the Republic of South Africa.
8. This Quotation Estimate is non-binding until both parties have signed and returned the acceptance section below. The Dietitian's professional services are governed by the HPCSA and SADA professional conduct rules.
By signing below, the client acknowledges and accepts this Quotation Estimate for Dietitian services as outlined above. The Dietitian confirms availability to commence services within Cape Town, South Africa, within ten (10) business days of receipt of the initial deposit.
