Payment for this Quotation Estimate shall be made via bank transfer to Bangkok Wellness Nutrition Co., Ltd. (Bank: Siam Commercial Bank, Account No. 012-3-45678-9, Branch: Sukhumvit, Bangkok). A 50% deposit is due upon acceptance of this Quotation Estimate, with the remaining 50% payable within 14 calendar days of service completion. All amounts are quoted in Thai Baht (THB) and are inclusive of applicable local taxes unless otherwise stated.
- This Quotation Estimate is valid for 30 days from the date of issue. Prices are subject to revision upon expiry.
- All Dietitian services are provided by registered and licensed practitioners recognized by the Thai Dietetic Association and the Thai Ministry of Public Health.
- Sessions conducted in Thailand Bangkok may be rescheduled up to 48 hours in advance without additional charge. Late cancellations may incur a 50% session fee.
- The Dietitian will not prescribe pharmaceutical medications. All nutritional recommendations are advisory and should be coordinated with the client's primary physician.
- Corporate workshops and on-site visits within the Bangkok metropolitan area are included in the quoted travel fee. Travel to provinces outside Bangkok will be quoted separately.
- Confidentiality of all client health records and dietary data is maintained in strict accordance with Thailand's Personal Data Protection Act (PDPA) B.E. 2562 (2019).
- This Quotation Estimate does not constitute a binding contract until formally accepted in writing by both parties via signed purchase order or service agreement.
- Any additional services beyond the scope outlined herein will be subject to a separate Quotation Estimate and mutual written approval.
