- This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. After the expiration date, pricing may be subject to revision based on market conditions in Indonesia Jakarta.
- Payment terms: 50% advance payment upon contract signing, with the remaining 50% payable in two equal monthly installments. All payments shall be made via bank transfer to the account designated by MediCare Jakarta Health Services PT.
- The Doctor General Practitioner assigned to this engagement must hold a valid SIP (Surat Izin Praktik) issued by the Jakarta City Health Office (Dinas Kesehatan DKI Jakarta) and be registered with the Indonesian Medical Council (IDI).
- MediCare Jakarta Health Services PT shall maintain comprehensive medical malpractice insurance coverage for all Doctor General Practitioner personnel deployed under this agreement.
- Confidentiality: All patient medical records and employee health data collected during the provision of Doctor General Practitioner services shall be treated in strict accordance with Indonesian Law No. 36 of 2009 on Health and the Personal Data Protection Law (UU PDP No. 27 of 2022).
- Either party may terminate this agreement with a written notice of sixty (60) days. In the event of early termination, a pro-rata refund of unused services will be calculated based on the original Quotation Estimate.
- This Quotation Estimate does not constitute a binding contract until both parties have executed the formal Service Agreement. All services are subject to availability of qualified Doctor General Practitioner personnel in Indonesia Jakarta.
- Any disputes arising from this engagement shall be resolved through mediation in accordance with the laws of the Republic of Indonesia, with jurisdiction vested in the Jakarta Central District Court.
By signing below, the client acknowledges receipt of this Quotation Estimate and agrees to the terms outlined herein for the provision of Doctor General Practitioner services in Indonesia Jakarta.
