- This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. Any services requested beyond this period may be subject to revised pricing by the Doctor General Practitioner and the clinic management.
- All medical services shall be rendered in accordance with the Malaysian Medical Council (MMC) Code of Ethics and the Private Healthcare Facilities and Services Act 1998 (Act 586) applicable in Malaysia Kuala Lumpur.
- The Doctor General Practitioner providing services under this Quotation Estimate holds a valid MMC registration number and is insured under a professional indemnity policy covering all clinical activities performed at the Kuala Lumpur premises.
- Payment terms: 50% advance payment upon acceptance of this Quotation Estimate; the remaining 50% shall be settled within fourteen (14) days of the completion of all contracted services for the billing period.
- All amounts are quoted in Malaysian Ringgit (MYR / RM). Service Tax (SST) at 6% is applicable as per the Royal Malaysian Customs Department regulations.
- The Doctor General Practitioner reserves the right to refer the patient to a specialist if the clinical condition exceeds the scope of general practice. Referral fees are included in Item 8 above.
- Confidentiality: All patient records and medical data shall be handled in strict compliance with the Personal Data Protection Act 2010 (PDPA) of Malaysia. No information shall be disclosed to third parties without written consent.
- This Quotation Estimate does not constitute a binding contract until signed and stamped by both parties. Upon acceptance, a formal Service Agreement will be executed.
- Any cancellation of scheduled appointments must be made at least 24 hours in advance. Late cancellations or no-shows may incur a 50% fee of the scheduled consultation rate.
- All services are to be conducted at the clinic premises located in Kuala Lumpur, Malaysia, unless otherwise agreed in writing for telehealth or on-site corporate visits.
