3.1 Validity: This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. Prices are subject to revision upon expiry due to fluctuations in the Nigerian Naira exchange rate and changes in pharmaceutical costs within Nigeria Abuja.
3.2 Payment Terms: A minimum deposit of 40% of the total amount is required to activate the service contract. The remaining balance shall be payable in two equal monthly instalments. All payments are to be made via bank transfer to the account of Abuja Medical & General Practice Centre, Access Bank, Account No. 0123456789, Abuja Branch, Nigeria.
3.3 Service Delivery: The Doctor General Practitioner shall be available for scheduled consultations Monday through Saturday, 8:00 AM to 6:00 PM, at the clinic premises in Maitama, Abuja. Home visits and urgent calls within the Nigeria Abuja metropolitan area will be arranged within four (4) hours of request.
3.4 Cancellation and Rescheduling: Appointments may be rescheduled with a minimum of 24 hours' notice. Cancellations made less than 24 hours before a scheduled visit will incur a 50% fee of the consultation charge.
3.5 Confidentiality: All patient records, medical histories, and health data collected by the Doctor General Practitioner are protected under the National Health Act 2014 and the Data Protection Regulation (Nigeria, 2019). No information will be disclosed to third parties without written consent.
3.6 Limitation of Liability: The Doctor General Practitioner provides general medical care and preventive services. In cases requiring specialist intervention, the patient will be referred to an appropriate specialist facility in Abuja. The Doctor General Practitioner is not liable for outcomes arising from non-compliance with prescribed treatment plans.
3.7 Dispute Resolution: Any disputes arising from this Quotation Estimate shall be resolved through mediation in accordance with the laws of the Federal Republic of Nigeria, with jurisdiction in Abuja, FCT.
Note: This Quotation Estimate does not constitute a binding contract until both parties have signed and the initial deposit has been received. All services are subject to the availability of the Doctor General Practitioner and the clinical appropriateness of the requested procedures.
