- This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. Beyond this period, the Doctor General Practitioner reserves the right to revise the fees in accordance with prevailing market rates and Ministry of Health Singapore Singapore regulatory adjustments.
- Payment is due within fourteen (14) days of the date of this Quotation Estimate or upon the completion of the respective service, whichever is earlier. Accepted payment methods include bank transfer, credit/debit card, PayNow, and cash.
- A 10% administrative fee will be applied to any cancellation of scheduled appointments made less than forty-eight (48) hours in advance. No fee applies to cancellations made more than forty-eight (48) hours prior to the appointment.
- All medical records, consultation notes, and diagnostic reports generated during the course of treatment by the Doctor General Practitioner remain the property of Meridian Health Clinic. Patients may request certified copies at a nominal administrative charge of SGD 10 per document.
- This Quotation Estimate does not constitute a guarantee of specific medical outcomes. The Doctor General Practitioner will exercise reasonable clinical judgement in determining the most appropriate course of treatment, which may result in additional or alternative services beyond those itemised herein.
- All services are subject to the Medical Council of Singapore Singapore regulations, the Health Professions Act, and the Personal Data Protection Act (PDPA) of Singapore Singapore. Patient confidentiality is maintained in strict accordance with these legal frameworks.
- In the event of any dispute arising from the services described in this Quotation Estimate, both parties agree to first attempt resolution through good-faith negotiation. Should resolution not be achievable, the matter shall be referred to the Singapore Mediation Centre for mediation before any legal proceedings are initiated.
- Meridian Health Clinic holds comprehensive medical malpractice insurance coverage. The Doctor General Practitioner is registered with the Singapore Medical Council and maintains valid professional indemnity insurance throughout the engagement period.
By signing below, the client acknowledges that they have read, understood, and accepted all terms and conditions set forth in this Quotation Estimate. The client authorises the Doctor General Practitioner at Meridian Health Clinic to provide the medical services as described herein in accordance with the Singapore Singapore healthcare standards and regulatory requirements.
