Important Notes
- This Quotation Estimate is valid for a period of thirty (30) calendar days from the date of issue. After the expiry date, pricing may be subject to revision based on prevailing market conditions in Thailand Bangkok.
- All fees quoted are in Thai Baht (THB) and include the applicable 7% Value Added Tax (VAT) as mandated by the Revenue Department of Thailand.
- The Doctor General Practitioner assigned to this engagement holds a valid medical license issued by the Thai Medical Council and carries professional indemnity insurance with a minimum coverage of 10,000,000 THB.
- Payment terms: 50% advance payment upon acceptance of this Quotation Estimate; the remaining 50% shall be settled within thirty (30) days of the completion of the contracted service period.
- Any services exceeding the quantities specified in this estimate will be billed at the unit rates listed above, subject to prior written approval from the client.
- This Quotation Estimate does not constitute a binding contract. A formal Service Agreement must be executed by both parties before any services commence.
- All medical services will be conducted in compliance with the Personal Data Protection Act (PDPA) of Thailand and applicable regulations of the Ministry of Public Health.
- The Doctor General Practitioner is not liable for pre-existing conditions that were not disclosed during the initial health assessment.
- Force majeure events, including but not limited to natural disasters, government-mandated closures, or public health emergencies affecting Thailand Bangkok, may result in service modifications or suspensions without penalty to either party.
- Disputes arising from this Quotation Estimate or the subsequent Service Agreement shall be resolved through amicable negotiation, failing which they shall be submitted to the competent courts of Bangkok, Thailand.
