Scope of Services – Midwife Care Package

This Quotation Estimate is prepared for the provision of comprehensive Midwife services to be delivered in Algeria Algiers. The following package encompasses all essential prenatal, intrapartum, and postnatal care to be administered by a certified and licensed Midwife operating in accordance with the Algerian Ministry of Health regulations and the professional standards established by the Algerian Order of Midwives (Ordre National des Sages-Femmes d'Algérie). All services will be rendered at the client's residence or at our dedicated clinic located in the Hydra district of Algeria Algiers, as specified by the client.

Terms and Conditions

This Quotation Estimate is valid for a period of thirty (30) calendar days from the date of issue. After the expiry date, all prices listed herein are subject to revision based on current market conditions in Algeria Algiers.
All Midwife services described in this document are provided by professionals holding valid licenses issued by the Algerian Ministry of Health and registered with the Algerian Order of Midwives. Proof of credentials will be provided upon request.
Payment terms: A deposit of 30% (DZD 74,613.00) is due upon acceptance of this Quotation Estimate. The remaining 70% shall be settled within seven (7) business days following the completion of all postnatal visits.
Payment may be made via bank transfer to the account of Algiers Maternal Care & Midwifery Services, BNA, Agence Hydra, IBAN: DZ76 0002 1010 0312 3456 7890 12, or by certified check payable to the company name.
In the event of a medical emergency requiring immediate transfer to a hospital in Algeria Algiers (e.g., CHU Mustapha, CHU Benbadis, or Polyclinique de Bab El Oued), the Midwife team will coordinate the transfer at no additional charge. Hospital fees, surgical procedures, and any specialized medical interventions are not included in this estimate and will be billed separately by the respective institution.
The client acknowledges that the Midwife services described herein are complementary to, and do not replace, the care provided by the client's primary obstetrician-gynecologist. All medical decisions remain under the authority of the attending physician.
Cancellation policy: Cancellation of scheduled Midwife appointments must be communicated at least 48 hours in advance. Cancellations within 48 hours will incur a 50% fee of the session price. Cancellation of the home birth attendance more than 14 days before the estimated due date will result in a full refund of the deposit; cancellation within 14 days will forfeit 50% of the deposit.
All personal health information collected during the provision of Midwife services in Algeria Algiers is protected under Algerian Law No. 18-07 relating to the protection of personal data. Information will not be disclosed to third parties without the client's written consent, except where required by law.
This Quotation Estimate does not constitute a binding contract until signed and countersigned by both parties. Upon acceptance, a formal service agreement will be executed detailing the full scope of the Midwife engagement.
Any disputes arising from the interpretation or execution of this document shall be resolved through amicable negotiation. Failing resolution, the competent courts of the jurisdiction of Algeria Algiers shall have exclusive jurisdiction.
