- This Quotation Estimate is prepared specifically for Midwife services delivered in Canada Montreal and its surrounding municipalities (Laval, Longueuil, and the South Shore). Travel beyond a 30-kilometre radius of downtown Montreal may incur an additional travel surcharge of $0.75 per kilometre.
- All Midwife services listed in this Quotation Estimate are covered in part or in full by the Régie de l'assurance maladie du Québec (RAMQ) for eligible residents. Our office will process all applicable Quebec health insurance claims on your behalf. The amounts shown represent the full professional fee before insurance reimbursement.
- Should your pregnancy present with high-risk complications (e.g., preeclampsia, gestational diabetes, multiple gestation), your Midwife will coordinate with an obstetrician at a Canada Montreal hospital. Additional specialist consultation fees are not included in this Quotation Estimate and will be quoted separately.

- A deposit of 20% of the total Quotation Estimate amount is required at the time of signing to secure your Midwife's availability. The remaining balance is payable in two installments: 50% at 36 weeks gestation and 50% within 14 days following delivery.
- All Midwife practitioners at our Canada Montreal clinic hold active registration with the Ordre professionnel des sages-femmes du Québec (OPSFQ) and carry professional liability insurance in accordance with Quebec regulations.
- Cancellation or rescheduling of Midwife appointments must be communicated at least 48 hours in advance. Late cancellations may be subject to a 50% fee of the scheduled visit.
