This Quotation Estimate is prepared for the provision of comprehensive Midwife services to be delivered within the metropolitan area of China Beijing. Note: This Quotation Estimate is specific to the Midwife services described herein and is valid for use within the administrative boundaries of China Beijing. Should the client require the Midwife to travel to a location outside the central urban districts of Beijing, an additional transportation surcharge of CNY 50 per kilometer beyond 15 km from the service center will apply. All prices are quoted in Chinese Yuan (CNY) and are inclusive of standard medical supplies used during Midwife consultations. Terms and Conditions

This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. After the expiration date, the Midwife service rates may be subject to revision based on prevailing market conditions in China Beijing.
Payment for the Midwife services shall be made in the following installments: 30% upon acceptance of this Quotation Estimate, 40% upon completion of all prenatal Midwife visits, and the remaining 30% within seven (7) days following the final postnatal Midwife visit.
The assigned Midwife shall maintain full professional liability insurance coverage as mandated by the Beijing Municipal Health Administration. A copy of the insurance certificate shall be provided to the client upon request.
All Midwife consultations and services shall be conducted in accordance with the latest clinical guidelines published by the Chinese Medical Association and the National Health Commission of the People's Republic of China.
Client confidentiality is strictly maintained in compliance with the Personal Information Protection Law of the People's Republic of China. All medical records generated during the Midwife engagement shall be stored securely and shall not be disclosed to third parties without written consent.
In the event of a medical emergency requiring transfer to a higher-level hospital in China Beijing, the Midwife shall coordinate the transfer and remain present until the client is safely admitted. Additional hospital fees are the responsibility of the client and are not included in this Quotation Estimate.
Cancellation of scheduled Midwife appointments must be communicated at least 48 hours in advance. Cancellations made within 48 hours will be subject to a 50% fee of the scheduled session rate.
This Quotation Estimate does not constitute a binding contract until both parties have signed and dated the acceptance section below. Upon signature, it shall serve as the governing agreement for the Midwife services rendered in China Beijing.
