This Quotation Estimate is issued by Addis Maternal & Child Care Services PLC to provide a comprehensive, itemized cost breakdown for the engagement of a certified Midwife to deliver full-spectrum maternal and newborn care services within the city of Ethiopia Addis Ababa. The scope of work outlined herein covers prenatal consultations, intrapartum (delivery) assistance, and postnatal follow-up care, all performed in accordance with the Federal Ministry of Health of Ethiopia clinical guidelines and the Ethiopian Midwifery Practice Standards (2023 revision). This document serves as a formal financial proposal and does not constitute a binding contract until countersigned by both parties.

The qualified Midwife assigned to this engagement shall hold a minimum of a Bachelor of Science in Midwifery (BSM) degree from an accredited Ethiopian university (e.g., Addis Ababa University College of Health Sciences or Jimma University) and a valid practicing license issued by the Ethiopian Health Professions Council. The Midwife shall provide the following services throughout the client's pregnancy and postpartum period in Ethiopia Addis Ababa:

*All prices are quoted in Ethiopian Birr (ETB) and are subject to adjustment should the Federal Democratic Republic of Ethiopia government revise the VAT rate or impose new regulatory levies on healthcare services prior to the delivery date.

This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. Beyond this period, pricing may be revised to reflect changes in input costs, fuel surcharges, or regulatory requirements applicable in Ethiopia Addis Ababa.
Payment Schedule: A non-refundable deposit of 30% (ETB 26,289.00) is due upon acceptance of this quotation. The remaining 70% (ETB 61,341.00) shall be paid within seven (7) days following the completion of postnatal care visits.
The assigned Midwife shall maintain strict confidentiality of all client medical records in compliance with the Ethiopian Health Sector Working Document and data protection regulations.
Cancellation by the client prior to the onset of labor entitles a refund of 50% of the deposit. Cancellation after labor has commenced is non-refundable.
This Quotation Estimate does not cover hospitalization fees, surgical interventions (e.g., cesarean section), or specialist consultations beyond the Midwife's scope of practice. Any such additional services will be quoted separately.
All services shall be rendered in accordance with the Ethiopian Midwifery Practice Standards and the Federal Ministry of Health's National Maternal and Child Health Strategy.
Disputes arising from this engagement shall be resolved through mediation in Addis Ababa, Ethiopia, in accordance with the Ethiopian Civil Code.

By signing below, the client acknowledges receipt of this Quotation Estimate and agrees to the terms outlined for the engagement of a Midwife to provide maternal and newborn care services in Ethiopia Addis Ababa. This document becomes a binding service agreement upon countersignature by both parties and receipt of the initial deposit.
