3.1 This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. After the expiry date, all prices listed herein are subject to revision in accordance with the prevailing fee schedule of the Jerusalem Maternal Care Center and the Israeli Ministry of Health guidelines.
3.2 Payment terms: 30% deposit (₪ 4,672.80) is due upon acceptance of this Quotation Estimate. The remaining 70% (₪ 10,903.20) is payable within fourteen (14) days following the completion of all midwifery services, including the final postpartum visit in Jerusalem.
3.3 All midwifery services are performed in compliance with the Israeli Health Professions Law (5733-1973) and the regulations issued by the Israel Nurses and Midwives Association. The assigned Midwife holds a valid practising licence issued by the Israeli Ministry of Health.
3.4 In the event of a high-risk pregnancy identified during antenatal care, the Midwife will coordinate with an obstetrician at Hadassah Medical Center, Jerusalem. Any additional specialist consultations will be quoted separately and are not included in this Quotation Estimate.
3.5 Cancellations of scheduled Midwife appointments must be made at least 48 hours in advance. Late cancellations or no-shows within the Jerusalem metropolitan area will incur a fee of 50% of the session rate.
3.6 This Quotation Estimate does not constitute a binding contract until signed and countersigned by both parties. The client acknowledges that all medical and midwifery information will be handled in strict accordance with the Israeli Privacy Protection Law (5741-1981).
3.7 The Midwife services described in this Quotation Estimate are limited to the geographic area of Jerusalem and its immediate surroundings within the State of Israel. Travel to locations outside this area will be subject to an additional transportation surcharge.
