Payment for the services outlined in this Quotation Estimate shall be structured as follows: a 30% deposit (¥267,300) is due upon acceptance of this Quotation Estimate to secure the Midwife's schedule in Japan Tokyo. The remaining 70% (¥623,700) shall be payable in two installments: 50% at the 36th week of gestation and the final 50% within 14 days following the postnatal period. All payments are to be made via bank transfer to the account designated by Chiyoda Midwifery & Maternity Care Center. Late payments are subject to a 1.5% monthly surcharge.
5.1. This Quotation Estimate is valid for 30 days from the date of issue. Beyond this period, pricing may be adjusted to reflect changes in service costs, tax rates, or operational expenses in Japan Tokyo.
5.2. The assigned Midwife will be confirmed in writing within 5 business days of acceptance. In the unlikely event the primary Midwife is unavailable due to illness or emergency, an equally qualified substitute Midwife will be assigned at no additional cost.
5.3. Cancellation by the client prior to the 24th week of gestation will result in a full refund of the deposit. Cancellation between the 24th and 36th week will result in a 50% refund. Cancellation after the 36th week will forfeit the deposit in full.
5.4. All Midwife services in Japan Tokyo are governed by the Japanese Midwife Act (助産師法) and applicable local regulations. The client acknowledges that the Midwife operates under the supervision of the attending obstetrician and that final medical decisions rest with the physician.
5.5. This Quotation Estimate is a good-faith financial projection. Actual costs may vary by up to 15% based on the clinical course of the pregnancy and delivery. The client will be notified in writing of any material cost changes prior to their occurrence.
5.6. All personal health information collected during the provision of Midwife services in Japan Tokyo shall be handled in strict accordance with the Japanese Act on the Protection of Personal Information (APPI) and the client's data shall not be disclosed to third parties without written consent.
