This Quotation Estimate is prepared by Golden Lotus Maternity & Midwifery Centre to provide a comprehensive and transparent cost breakdown for professional Midwife services to be delivered in Myanmar Yangon. The purpose of this document is to outline all anticipated charges, service inclusions, and applicable terms so that the client may make an informed decision regarding the engagement of our certified Midwife team for prenatal, intrapartum, and postnatal care. All pricing in this Quotation Estimate is quoted in Myanmar Kyat (MMK) and reflects the current market rates for qualified Midwife services in the Myanmar Yangon metropolitan area as of the date of issue.

The following Midwife services are included in this Quotation Estimate and are to be performed at our facility located in Kamayut Township, Myanmar Yangon, or at the client's private residence within the Yangon Region upon prior arrangement:

• Prenatal Midwife Consultations: A minimum of eight (8) scheduled antenatal visits conducted by a licensed Midwife registered with the Myanmar Nursing Council, including fetal heart monitoring, blood pressure assessment, nutritional counselling, and preparation for safe delivery in Myanmar Yangon.
• Labour and Delivery Support: Continuous one-on-one Midwife attendance during the entire labour and delivery process, including pain management guidance, positioning assistance, and emergency escalation protocols in coordination with the on-site obstetric team.
• Postnatal Midwife Care: Four (4) postnatal home visits within the first two weeks after delivery, covering newborn assessment, breastfeeding support, maternal wound care, and mental health screening.
• Newborn Care Package: Umbilical cord care, jaundice monitoring, and initial immunisation guidance provided by the assigned Midwife.
• 24/7 Emergency Midwife Hotline: Access to an on-call Midwife via telephone for the duration of the pregnancy and the first six weeks postpartum.

The total amount stated in this Quotation Estimate shall be paid as follows:

• 30% Advance Payment (MMK 362,250) due upon acceptance of this Quotation Estimate and signing of the service agreement.
• 40% Mid-Payment (MMK 483,000) due at the 36th week of pregnancy, prior to the scheduled delivery date.
• 30% Final Payment (MMK 362,250) due within seven (7) calendar days following the completion of all postnatal Midwife visits.

Payments may be made via bank transfer to Golden Lotus Maternity & Midwifery Centre, CB Bank, Yangon Branch, Account No. 001-2345-6789, or by cash receipt at our Myanmar Yangon office. All payments are non-refundable once the Midwife has been assigned and scheduled.

• This Quotation Estimate is valid for thirty (30) days from the date of issue. After the expiry date, pricing may be revised to reflect changes in operational costs in Myanmar Yangon.
• All Midwife professionals engaged under this quotation hold valid licences issued by the Myanmar Nursing Council and carry professional indemnity insurance.
• In the event of a medical emergency requiring transfer to a tertiary hospital (e.g., Yangon General Hospital or 700-Bed Hospital), additional costs for surgical intervention, anaesthesia, and inpatient care are not included in this Quotation Estimate and will be billed separately.
• The client agrees to provide accurate medical history and to attend all scheduled Midwife appointments. Missed appointments must be rescheduled at least 48 hours in advance.
• This Quotation Estimate does not constitute a binding contract until both parties have signed and dated the attached Service Agreement.
• All services are subject to the regulations of the Myanmar Ministry of Health and Sports and applicable local by-laws in Yangon Region.
