- This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. Beyond this period, all rates are subject to revision based on prevailing economic conditions in Nigeria Lagos.
- The Midwife assigned to this engagement shall be a fully registered and licensed practitioner with the Nursing and Midwifery Council of Nigeria (NMC) and shall carry valid professional indemnity insurance throughout the service period.
- Payment terms: A non-refundable deposit of 40% (₦610,600) is due upon acceptance of this Quotation Estimate. The remaining 60% shall be payable in two equal instalments: 30% at the commencement of antenatal care and 30% within seven (7) days following the postnatal period.
- All services shall be rendered within the Nigeria Lagos metropolitan area. Travel to locations outside the defined service radius (Lekki, Victoria Island, Ikoyi, Ajah, and surrounding communities) shall attract an additional transportation surcharge of ₦10,000 per trip.
- The Midwife shall adhere to the clinical protocols established by the Federal Ministry of Health, Nigeria, and the Lagos State Ministry of Health guidelines for maternal and neonatal care.
- In the event of a medical emergency requiring transfer to a tertiary hospital (e.g., LUTH, Reddington Hospital, or Lagoon Hospital), the Midwife shall coordinate the transfer at no additional cost to the client. Hospital admission fees and surgical interventions are excluded from this Quotation Estimate.
- Confidentiality: All patient records and personal information shall be treated in strict accordance with the Nigeria Data Protection Regulation (NDPR) 2019. No information shall be disclosed to third parties without the client's written consent.
- Cancellation: Should the client wish to cancel the engagement, a written notice of at least fourteen (14) days is required. The 40% deposit is non-refundable; any additional fees already incurred for services rendered shall remain payable.
- This Quotation Estimate does not constitute a binding contract until signed and countersigned by both parties. A formal Service Agreement shall be executed upon acceptance.
