1. Introduction & Purpose of This Quotation EstimateThis Quotation Estimate is formally issued by Capital Maternal Care Services (Pvt.) Ltd., a licensed and registered healthcare provider operating in Pakistan Islamabad, to present a comprehensive and transparent cost breakdown for professional Midwife services. This document serves as a binding financial proposal for the client and outlines all deliverables, associated costs, timelines, and terms governing the engagement. The Midwife services described herein are designed to support expectant mothers throughout the antenatal, intrapartum, and postnatal periods in full compliance with the standards set by the Pakistan Nursing & Midwifery Council and the Federal Ministry of National Health Services, Regulations & Coordination.
2. Scope of Midwife ServicesThe following Midwife services are included in this Quotation Estimate for delivery at the client's residence or a designated facility within Pakistan Islamabad:

- This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. Beyond this period, prices may be revised to reflect changes in operational costs within Pakistan Islamabad.
- Payment Terms: 40% advance payment upon acceptance of this Quotation Estimate, 40% upon completion of the antenatal phase, and the remaining 20% within seven (7) days of the postnatal period conclusion.
- All Midwife professionals assigned to this engagement hold valid registration with the Pakistan Nursing & Midwifery Council (PNMC) and possess a minimum of five (5) years of clinical experience in obstetric and neonatal care.
- The Midwife service is strictly limited to the geographical boundaries of the Islamabad Capital Territory (ICT), including Rawalpindi, Bahria Town, DHA, F-10, F-6, G-11, and adjacent sectors within Pakistan Islamabad.
- In the event of a medical emergency requiring hospital transfer, the Midwife will coordinate with the nearest tertiary care facility. Hospital admission charges, surgical fees, and ICU costs are not included in this Quotation Estimate and will be borne separately by the client.
- Cancellation Policy: Cancellation more than 14 days before the expected delivery date will incur a 15% administrative fee. Cancellation within 14 days will incur a 50% fee. No refund is applicable after labor has commenced.
- All services are subject to the client providing a safe, clean, and adequately lit environment for the Midwife to perform her duties in accordance with infection control protocols mandated by the Islamabad Health Department.
- This Quotation Estimate does not constitute a medical diagnosis or treatment plan. All clinical decisions remain under the supervision of the attending obstetrician/gynecologist.
- Confidentiality: All patient records, health data, and personal information are protected under the Pakistan Personal Data Protection framework and will not be disclosed to third parties without written consent.
By signing below, the client acknowledges receipt of this Quotation Estimate and agrees to the terms, conditions, and pricing outlined for the Midwife services to be rendered in Pakistan Islamabad.
