The following services are included in this Quotation Estimate for the engagement of a licensed Midwife in Peru Lima. Our Midwife is registered with the Colegio de Obstetras del Perú and holds a valid professional license issued by the Ministry of Health (MINSA) of Peru. All services are conducted in compliance with the national regulations governing midwifery practice in Peru Lima.

This Quotation Estimate is valid for a period of thirty (30) calendar days from the date of issuance. Should the client require an extension, a revised Quotation Estimate may be issued upon written request. All fees listed are inclusive of the Midwife's professional time, standard medical supplies, and documentation. Any additional services not explicitly listed herein, such as cesarean section assistance, neonatal resuscitation beyond standard protocols, or referral to a hospital in Peru Lima, will be subject to a supplementary Quotation Estimate and prior written consent from the client.

The Midwife assigned to this engagement will operate under the supervision and in coordination with the client's attending obstetrician, should one be designated. In the event that the Midwife determines that the pregnancy presents high-risk factors requiring hospital-based delivery in Peru Lima, the Midwife will facilitate the transfer and provide continuous support throughout the process. The costs associated with hospital admission, surgical procedures, and inpatient care are not included in this Quotation Estimate and will be billed separately by the respective medical facility.

• Deposit: A non-refundable deposit of 30% (PEN 3,057.00) of the total Quotation Estimate amount is due upon acceptance of this document to secure the Midwife's schedule in Peru Lima.
• Installment 1: 30% (PEN 3,057.00) due at the completion of the fourth prenatal visit.
• Installment 2: 30% (PEN 3,057.00) due at the completion of the eighth prenatal visit or two weeks prior to the estimated due date, whichever comes first.
• Final Payment: 10% (PEN 1,019.00) due within seven (7) business days following the final postpartum visit.
• Payment may be made via bank transfer to the account of Materna Lima Midwifery Center (BIC: BCPEPE2L, Account: 0000-000000000000) or by certified check. All transactions are subject to the applicable Peruvian tax regulations (IGV 18% included in the above figures).

• This Quotation Estimate does not constitute a binding contract until both parties have signed and dated the acceptance section below.
• The Midwife reserves the right to decline or modify services if, during the course of care, the clinical situation in Peru Lima necessitates a change in the care plan for the safety of the mother and newborn.
• Cancellation by the client after the deposit has been paid will result in the forfeiture of the deposit. Cancellation before the deposit is due will incur no obligation.
• The Midwife is covered by professional liability insurance (póliza de responsabilidad civil profesional) valid throughout Peru Lima and the broader Lima metropolitan region.
• All medical records and documentation generated during the engagement will be maintained in accordance with Peruvian data protection law (Ley N° 29733) and the client's privacy rights.
• Disputes arising from this Quotation Estimate shall be resolved through mediation in Peru Lima, in accordance with the Civil Code of Peru.

By signing below, the client acknowledges receipt of this Quotation Estimate, confirms understanding of the scope of Midwife services described herein, and agrees to the payment terms and conditions outlined in this document for services to be rendered in Peru Lima.
