Terms Governing This Quotation Estimate for Midwife Services in Johannesburg, South Africa
- This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. After this period, pricing for the Midwife services may be subject to revision in line with South African healthcare cost adjustments.
- Payment is due within fourteen (14) days of invoice issuance. The client may elect to pay via EFT to the registered Midwife's practice account in Johannesburg, or through direct medical aid billing with Discovery Health.
- A deposit of 20% (R 7,340.00) is required to secure the Midwife's availability for the full antenatal, intrapartum, and postnatal care package within the Johannesburg metropolitan area.
- All Midwife services are subject to the client's medical aid policy terms. Any co-payment, excess, or non-covered items will be communicated in writing prior to service delivery.
- The Midwife reserves the right to refer the client to a specialist obstetrician or gynaecologist in Johannesburg should clinical indications require it. Such referrals are included in this Quotation Estimate at no additional administrative cost.
- Cancellation of scheduled Midwife appointments must be communicated at least 48 hours in advance. Late cancellations within the Johannesburg service area will incur a 50% fee of the scheduled session cost.
- All clinical records and documentation generated during the provision of Midwife services in South Africa are maintained in accordance with the Protection of Personal Information Act (POPIA, Act 4 of 2013) and the HPCSA record-keeping standards.
- This Quotation Estimate does not constitute a contract of service. A formal service agreement will be executed upon acceptance of this estimate by the client.
- Any disputes arising from this Quotation Estimate shall be resolved in accordance with the laws of the Republic of South Africa, and the competent courts of Johannesburg shall have exclusive jurisdiction.
