1. Scope of Midwife Services

2. Payment Terms
This Quotation Estimate is structured with the following payment schedule to ensure continuity of Midwife care throughout your pregnancy and postnatal period in Sri Lanka Colombo:
- Advance Payment (40%): LKR 117,400 — Due upon acceptance of this quotation and signing of the service agreement.
- Mid-Pregnancy Payment (30%): LKR 88,050 — Due at the 32nd week of gestation.
- Final Balance (30%): LKR 88,050 — Due within 7 days following the completion of postnatal midwife visits.
Payments may be made via bank transfer to our account at Commercial Bank of Ceylon, Colombo Branch, or by certified cheque. All amounts are quoted in Sri Lankan Rupees (LKR) and are inclusive of the prevailing 18% Value Added Tax as mandated by the Inland Revenue Department of Sri Lanka.
3. Terms & Conditions
- This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. Beyond this period, pricing may be revised to reflect changes in operational costs in Sri Lanka Colombo.
- All Midwife services are to be delivered by a registered midwife holding a valid licence issued by the Nursing and Midwifery Council of Sri Lanka. The assigned midwife will be Ms. Sanduni Jayasinghe, RN, RM (Reg. No: NMCS/2019/0452).
- The client agrees to provide accurate medical history and to disclose any pre-existing conditions that may affect the midwife's care plan. Failure to do so may result in service modification or termination.
- In the event of a medical emergency requiring hospital intervention, the midwife will coordinate the transfer to the nearest appropriate facility in Colombo. Hospital charges, surgical fees, and specialist consultations are not included in this quotation estimate and will be the sole responsibility of the client.
- Cancellation by the client after the 28th week of gestation will incur a non-refundable fee of 50% of the total quoted amount. Cancellation before the 28th week will incur a 20% administrative fee.
- All services will be conducted in compliance with the Sri Lankan Maternal Health Act and the regulations of the Ministry of Health, Sri Lanka. The midwife will maintain strict confidentiality of all patient records in accordance with Sri Lankan data protection guidelines.
- This quotation estimate does not constitute a guarantee of a specific delivery outcome. The midwife will exercise all reasonable professional judgement to ensure the safety and well-being of both mother and child.
- Any additional services not listed herein (e.g., caesarean section, neonatal intensive care, or extended postnatal support) will be quoted separately in writing prior to commencement.
4. Acceptance
By signing below, the client acknowledges receipt of this Quotation Estimate and agrees to the terms and conditions outlined above for the provision of Midwife services in Sri Lanka Colombo. This document, once countersigned by both parties, shall serve as a binding service agreement.
