This Quotation Estimate has been prepared by Zürich Midwifery Practice AG to provide Ms. Anna Keller with a comprehensive and transparent financial overview of all Midwife services to be rendered during the antenatal, intrapartum, and postnatal periods. As a registered and licensed Midwife operating under the Swiss Federal Act on Medical Professions (MedBG), our practice is fully accredited to deliver evidence-based perinatal care within the canton of Zurich and the broader Switzerland Zurich metropolitan area. This document serves as a binding cost estimate and does not constitute a final invoice. All figures are quoted in Swiss Francs (CHF) and are subject to the terms outlined in Section 6 of this Quotation Estimate.

This Quotation Estimate includes all standard Midwife clinical supplies (sterile gloves, fetal doppler, blood pressure cuff, newborn scales, and basic resuscitation equipment) used during the delivery in Switzerland Zurich. The following are not included and will be billed separately if required: obstetric emergency interventions, transfer to a Zurich hospital (e.g., Universitätsspital Zürich or Kantonsspital), neonatal intensive care, additional laboratory tests beyond the standard panel, and any services necessitated by a high-risk pregnancy classification. In the event of a hospital transfer, the Midwife will coordinate seamlessly with the on-call obstetric team, and the client will be provided with a supplementary Quotation Estimate for any additional costs incurred.
Payment for the services outlined in this Quotation Estimate is structured as follows: a deposit of 20% (CHF 1,372.00) is due upon acceptance of this document; 50% (CHF 3,430.00) is due at 36 weeks of gestation; and the remaining 30% (CHF 2,058.00) is due within 14 days following the postnatal period. All payments are to be made via bank transfer to Zürich Midwifery Practice AG, IBAN CH93 0076 2011 6238 5295 7, BIC POFICHBEXXX. The client is encouraged to submit the relevant Midwife service invoices directly to their Swiss health insurance provider for reimbursement under the mandatory basic insurance (Krankentaggversicherung) as stipulated by the Federal Health Insurance Act (KVG).
- This Quotation Estimate is valid for 30 calendar days from the date of issue. After the expiry date, a revised Quotation Estimate may be required to reflect any changes in service pricing or availability in Switzerland Zurich.
- The Midwife reserves the right to modify the scope of services if the client's medical condition changes, in which case a supplementary Quotation Estimate will be issued and must be accepted in writing before additional services commence.
- All Midwife services are governed by the Swiss Code of Obligations (OR) and the professional code of ethics of the Swiss Association of Midwives. Disputes shall be resolved under the jurisdiction of the courts in Switzerland Zurich.
- Client data is processed in strict accordance with the Swiss Federal Act on Data Protection (DSG) and the revised GDPR provisions applicable in Switzerland Zurich.
- Cancellation by the client after 32 weeks of gestation will result in a non-refundable fee of 50% of the total Quotation Estimate value.
- This Quotation Estimate does not guarantee a specific mode of delivery. The Midwife will always act in the best clinical interest of the mother and child, which may include recommending or facilitating a hospital transfer.
