This Quotation Estimate is issued by London Midwifery & Maternity Care Services Ltd. to provide a comprehensive and itemised financial estimate for the provision of professional Midwife services to the named client. All services described herein shall be delivered in accordance with the standards set by the Nursing and Midwifery Council (NMC) of the United Kingdom and in full compliance with the regulations governing maternity care in United Kingdom London. This Quotation Estimate covers the full continuum of antenatal, intrapartum, and postnatal care that our registered Midwife will provide to the client throughout her pregnancy and the immediate postpartum period.
The Midwife assigned to this engagement, Ms. Sarah Okafor (NMC PIN: 1234567), is a fully registered and practising Midwife with over twelve years of clinical experience in both NHS and private maternity units across the London metropolitan area. She holds a BSc (Hons) in Midwifery from King's College London and a Postgraduate Diploma in Perinatal Mental Health. All services will be conducted at the client's residence in Kensington, London, or at our private maternity suite located in Marylebone, United Kingdom London, as mutually agreed upon in writing prior to each appointment.

Note: Certain Midwife services provided as part of NHS-funded care may be exempt from VAT under UK legislation. The final VAT treatment will be confirmed at the point of invoicing. This Quotation Estimate assumes a standard, low-risk pregnancy. Any additional specialist referrals, caesarean section, or extended postnatal care will be subject to a supplementary Quotation Estimate issued separately.
The total amount specified in this Quotation Estimate shall be payable in accordance with the following schedule: a deposit of 25% (£1,845.00) is due upon acceptance of this Quotation Estimate to secure the Midwife's availability; 50% (£3,690.00) is due by the 32nd week of pregnancy; and the remaining 25% (£1,845.00) is due within fourteen (14) calendar days following the postnatal discharge visit. Payment may be made by direct bank transfer to our London office account, by debit or credit card, or by cheque made payable to "London Midwifery & Maternity Care Services Ltd."
- This Quotation Estimate is valid for thirty (30) days from the date of issue. After the expiry date, all rates are subject to revision in line with prevailing market conditions in United Kingdom London.
- All Midwife services are subject to the Midwife's clinical judgement. Should the Midwife determine that the client's condition requires transfer to a hospital setting or referral to an obstetrician, the client will be informed immediately, and any additional costs will be itemised in a supplementary Quotation Estimate.
- The Midwife is a self-employed practitioner operating under the professional indemnity insurance of London Midwifery & Maternity Care Services Ltd. (Policy No. PI-2025-78432, cover limit £10,000,000). The Midwife is not an employee of the client and operates under an independent contractor agreement.
- Cancellation of any scheduled Midwife appointment must be made no fewer than forty-eight (48) hours in advance. Late cancellations or no-shows will be charged at 50% of the applicable session fee.
- All clinical records, Midwife notes, and personal data will be handled in strict accordance with the UK General Data Protection Regulation (UK GDPR) 2018 and the Data Protection Act 2018. Records will be retained for a minimum of eight (8) years in line with NMC guidance.
- This Quotation Estimate does not constitute a contract until signed and countersigned by both parties. Upon acceptance, a formal Service Agreement will be issued within five (5) working days.
- All services will be delivered within the Greater London area, United Kingdom. Travel to locations outside the M25 orbital motorway will incur an additional mileage charge of £0.65 per mile, to be detailed in a supplementary Quotation Estimate.
- Disputes arising from this Quotation Estimate or the services described herein shall be resolved in accordance with the laws of England and Wales, and the exclusive jurisdiction of the courts of London, United Kingdom shall apply.
