The Nurse engaged under this Quotation Estimate will be a Registered Nurse (RN) holding an active Australian Health Practitioner Regulation Agency (AHPRA) registration, with a minimum of five (5) years of post-qualification clinical experience in community and home-based care settings within Australia Brisbane. The Nurse will be employed under the direct supervision of our Clinical Director and will operate in full compliance with the Nursing and Midwifery Board of Australia (NMBA) Code of Conduct and all Queensland Health regulatory requirements.

The services to be delivered include, but are not limited to: daily wound assessment and dressing changes; medication administration and reconciliation; intravenous (IV) therapy monitoring; vital signs monitoring and documentation; patient and family education regarding post-operative recovery; coordination with the patient's General Practitioner (GP) and specialist team; and provision of a written clinical progress report to the client on a weekly basis.

This Quotation Estimate proposes an initial engagement period of three (3) months, commencing on 1 July 2025, subject to written acceptance by the client. The Nurse will conduct an initial comprehensive clinical assessment at the client's residence in Toowong, Brisbane, on the first day of service. Following the initial assessment, the Nurse will develop a personalised care plan in consultation with the client, their family members, and the treating medical team. The engagement may be extended on a month-to-month basis upon mutual written agreement.

• All invoices under this Quotation Estimate are payable within fourteen (14) calendar days of the date of issue.
• Payment may be made via direct bank transfer (EFT) to the company's nominated account, or by credit card (Visa, Mastercard, or AMEX).
• A 2% late payment surcharge will apply to any invoice outstanding beyond the 14-day payment window, in accordance with the Queensland Fair Trading Act 2010.
• For clients eligible for Home Care Package (HCP) funding under the Australian Government's My Aged Care program, the Nurse's fees may be partially or fully offset against the client's allocated HCP funds. Our office will assist with all necessary paperwork and claims processing.
• Private health insurance rebates, where applicable, will be processed directly by the insurer and are not deducted from the amounts stated in this Quotation Estimate.

• This Quotation Estimate is valid for thirty (30) days from the date of issue. After this period, pricing may be subject to revision based on prevailing wage conditions, fuel costs, and market rates within the Australia Brisbane healthcare sector.
• The Nurse assigned to this engagement will undergo a current National Police Check, Working with Children Check (if applicable), and annual health screening (influenza and tetanus vaccinations) as required by Queensland Health and our internal compliance policy.
• The client agrees to provide a safe and accessible environment for the Nurse to perform clinical duties. Any hazards or safety concerns must be reported to our office prior to the Nurse's arrival.
• Either party may terminate the engagement with fourteen (14) days' written notice. In the event of early termination by the client without cause, a cancellation fee equivalent to one week's nursing fees will be applied.
• All clinical records, care plans, and documentation generated by the Nurse will be stored in accordance with the Privacy Act 1988 (Cth) and the NMBA's record-keeping standards. Records will be retained for a minimum of seven (7) years.
• This Quotation Estimate does not constitute a medical diagnosis or a guarantee of specific clinical outcomes. The Nurse will exercise independent clinical judgement in the delivery of care, guided by evidence-based practice and the NMBA Standards for Professional Practice.
• Any disputes arising from this engagement will be resolved in accordance with the Queensland Civil and Administrative Tribunal (QCAT) procedures or, where applicable, through the Australian Health Care Complaints Authority (AHCA) process.
