The Nurse services outlined in this Quotation Estimate are designed to meet the specific clinical and personal care requirements of the client as discussed during the initial assessment conducted on 28 May 2025 at the client's residence in Mosman, Sydney. All Nurse personnel engaged under this Quotation Estimate hold current Australian Health Practitioner Regulation Agency (AHPRA) registration and are fully insured for practice in Australia Sydney.

The following items are excluded from this Quotation Estimate and will be billed separately if required:
• Consumable medical supplies (dressing packs, catheter kits, IV fluids, etc.) — billed at cost plus 15% handling fee.
• Travel surcharge for Nurse visits outside the Greater Sydney 20 km radius — $0.85 per km.
• After-hours and public holiday Nurse attendance — 1.5x the standard hourly rate.
• Any specialist referrals, diagnostic imaging, or hospital admission costs incurred during the Nurse care period.
4.1 This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. After the expiry date, all Nurse service rates are subject to review and may be adjusted in accordance with the Australian Nursing and Midwifery Federation (ANMF) award rates applicable in Australia Sydney.
4.2 Payment is due within fourteen (14) days of the monthly invoice date. Late payments will attract interest at the rate of 2% per month as permitted under the Australian Consumer Law.
4.3 The Nurse assigned to this engagement will be a registered practitioner with a minimum of five (5) years of post-registration clinical experience in Australia Sydney. The client reserves the right to request a Nurse substitution with reasonable notice (minimum 48 hours).
4.4 All Nurse services are delivered in compliance with the National Safety and Quality Health Service (NSQHS) Standards and the Privacy Act 1988 (Cth). Client health records are stored in accordance with Australian data protection legislation.
4.5 This Quotation Estimate may be terminated by either party with a written notice period of fourteen (14) days. No penalty applies for early termination beyond the notice period.
4.6 Sydney Allied Health & Nursing Solutions Pty Ltd maintains Professional Indemnity Insurance (minimum $20 million) and Public Liability Insurance (minimum $20 million) covering all Nurse operations in Australia Sydney.
