This Quotation Estimate document has been prepared by Dhaka Care & Wellness Services Ltd. to provide a comprehensive and transparent cost breakdown for the engagement of a qualified Nurse for in-home and hospital-based care services within the metropolitan area of Bangladesh Dhaka. This estimate covers all professional fees, equipment costs, and ancillary services associated with the deployment of a registered Nurse to the client's residence or designated medical facility in Bangladesh Dhaka for a period of twelve (12) months, commencing from the date of acceptance.
The Nurse services outlined in this Quotation Estimate are designed to meet the specific healthcare needs of the client and their family members residing in Bangladesh Dhaka. The scope includes but is not limited to the following professional duties performed by the assigned Nurse:
- Daily vital sign monitoring (blood pressure, temperature, pulse, respiratory rate, and oxygen saturation) for elderly or post-surgical patients.
- Medication administration and management in strict accordance with physician-prescribed schedules.
- Wound care, dressing changes, and catheter management as directed by the attending physician.
- Assistance with mobility, personal hygiene, and daily living activities for bedridden or partially mobile patients.
- Preparation and administration of injectable medications, IV fluids, and subcutaneous injections.
- Maintenance of detailed patient care logs and periodic health reports submitted to the family and primary care physician.
- Emergency first-aid response and coordination with local hospitals in Bangladesh Dhaka in case of medical emergencies.
- Nutritional guidance and dietary monitoring in consultation with a registered dietitian.

4.1 This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. Beyond this period, all rates and availability of the Nurse may be subject to revision based on market conditions in Bangladesh Dhaka.
4.2 The assigned Nurse must hold a valid registration with the Bangladesh Nursing and Midwifery Council (BNMC) and possess a minimum of five (5) years of clinical experience in geriatric or post-operative care.
4.3 Payment terms: A 50% advance payment is due upon acceptance of this Quotation Estimate. The remaining 50% shall be paid in equal monthly installments on or before the 5th of each calendar month.
4.4 The Nurse shall work on a 24/7 in-residence basis with a mandatory 8-hour rest period per 24-hour cycle. Overtime or additional shifts beyond the agreed schedule will be billed at 1.5x the hourly rate.
4.5 In the event the client requires a replacement Nurse due to illness, leave, or performance concerns, Dhaka Care & Wellness Services Ltd. shall provide a qualified substitute within 48 hours at no additional cost.
4.6 All medical records and patient data handled by the Nurse in Bangladesh Dhaka shall be treated with strict confidentiality in compliance with the Digital Security Act 2018 and applicable data protection regulations of Bangladesh.
4.7 This Quotation Estimate does not include the cost of prescription medications, surgical procedures, or specialist consultations, which shall be billed separately by the respective providers.
4.8 Cancellation by the client after the first 30 days of service will incur a penalty of one (1) month's full Nurse fee. Cancellation by our company will be subject to a 14-day written notice.
Note: This Quotation Estimate is prepared exclusively for the named client and is non-transferable. All services will be delivered within the administrative boundaries of Bangladesh Dhaka (Dhaka Division). For services extending to Chittagong, Sylhet, or other divisions, a separate Quotation Estimate will be required.By signing below, the client acknowledges that they have reviewed this Quotation Estimate in its entirety, understand the scope of Nurse services to be provided in Bangladesh Dhaka, and agree to the terms, conditions, and financial obligations outlined herein.
