This Quotation Estimate is issued by CareBridge Healthcare Services Pvt. Ltd. for the provision of professional Nurse services to be delivered at the client's residential premises in India Bangalore. The Nurse assigned to this engagement will be a Registered Nurse (RN) holding a valid license from the Karnataka State Nursing Council and the Indian Nursing Council (INC). The scope of services includes but is not limited to: daily vital sign monitoring (blood pressure, pulse, temperature, respiratory rate, and SpO2), medication administration and management, wound dressing and care, post-surgical recovery support, mobility assistance, nutritional guidance, and comprehensive patient observation with daily written reports submitted to the client or attending physician. The Nurse will be available on a 24-hour shift basis as specified below, ensuring continuous and compassionate care within the India Bangalore metropolitan area.

• Validity: This Quotation Estimate is valid for a period of thirty (30) calendar days from the date of issue. Beyond this period, rates may be revised based on prevailing market conditions in India Bangalore.
• Payment Terms: 50% advance payment is required to confirm the Nurse assignment. The remaining 50% shall be payable within seven (7) days of the completion of the 30-day service period. Payments may be made via bank transfer, UPI, or demand draft to the account specified in the invoice.
• Nurse Replacement: In the event the assigned Nurse is unable to continue due to illness or personal reasons, CareBridge Healthcare will deploy a replacement Nurse of equivalent or higher qualification within 12 hours at no additional cost to the client.
• Service Hours: The Nurse will work in 12-hour shifts (12 AM – 12 PM and 12 PM – 12 AM) with a 30-minute break. Overtime beyond the contracted hours will be billed at ₹250 per hour.
• Cancellation Policy: Cancellation with more than 7 days' notice will incur a 10% penalty on the total Quotation Estimate amount. Cancellation with less than 7 days' notice will incur a 25% penalty.
• Confidentiality: All patient information, medical records, and personal data handled by the Nurse will be treated with strict confidentiality in compliance with the Digital Personal Data Protection Act, 2023 (India).
• Liability: CareBridge Healthcare Services Pvt. Ltd. shall not be liable for any pre-existing medical conditions or complications arising from the patient's underlying health status. The Nurse will act in accordance with standard medical protocols and will escalate any critical situations to the attending physician immediately.
• Jurisdiction: This Quotation Estimate and any disputes arising therefrom shall be governed by the laws of India, with exclusive jurisdiction of the courts in Bangalore, Karnataka.

By signing below, the client acknowledges receipt of this Quotation Estimate for Nurse services in India Bangalore and agrees to the terms, conditions, and pricing outlined herein. This signature constitutes a binding acceptance of the Quotation Estimate and authorizes CareBridge Healthcare Services Pvt. Ltd. to proceed with the Nurse deployment as described.
