The Nurse professional engaged under this Quotation Estimate shall provide the following services exclusively within the Indonesia Jakarta service area, including but not limited to South Jakarta, North Jakarta, Central Jakarta, East Jakarta, and West Jakarta administrative districts:
- Clinical Care Administration: The Nurse shall perform wound dressing, intravenous (IV) therapy administration, medication management, vital sign monitoring, and post-surgical recovery support at the client's designated residence or facility in Indonesia Jakarta.
- Patient Assessment & Reporting: Daily clinical assessments, documentation of patient progress, and weekly summary reports submitted to the supervising physician. The Nurse shall maintain all records in accordance with Indonesian medical data privacy law (UU No. 17/2023).
- Chronic Disease Management: Ongoing monitoring and care coordination for patients with diabetes, hypertension, cardiac conditions, or respiratory disorders, as directed by the attending physician in Indonesia Jakarta.
- Emergency Response Coordination: The Nurse shall be trained in Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) protocols and shall coordinate with the nearest emergency medical services in Indonesia Jakarta in the event of a medical emergency.

- This Quotation Estimate is valid for a period of thirty (30) calendar days from the date of issue. Beyond this period, all rates for the Nurse service in Indonesia Jakarta are subject to revision based on prevailing market conditions and regulatory adjustments.
- Payment terms: 50% advance payment upon acceptance of this Quotation Estimate, and the remaining 50% due within seven (7) business days following the completion of the 30-day Nurse service period in Indonesia Jakarta.
- All payments shall be made via bank transfer to the designated account of MediCare Professional Services Indonesia. No cash payments for the Nurse engagement will be accepted.
- The Nurse assigned under this arrangement shall hold a valid STR (Surat Tanda Registrasi) issued by the Indonesian Ministry of Health and a current PPNI membership card. Copies of credentials shall be provided prior to commencement of service in Indonesia Jakarta.
- Any additional services beyond the scope defined in this Quotation Estimate shall be subject to a separate written amendment and additional Quotation Estimate prior to execution.
- This Quotation Estimate does not constitute a service contract. A formal Service Level Agreement (SLA) and employment/engagement contract for the Nurse shall be executed separately upon mutual acceptance.
- Force majeure events, including but not limited to natural disasters, government-mandated lockdowns, or public health emergencies affecting Indonesia Jakarta, may result in service interruption without penalty to either party.
By signing below, the client acknowledges receipt and acceptance of this Quotation Estimate for the Nurse service engagement in Indonesia Jakarta. This signature authorizes MediCare Professional Services Indonesia to proceed with Nurse recruitment, credentialing, and deployment as outlined herein.
