The Nurse(s) assigned under this Quotation Estimate will meet the following minimum criteria: a minimum of three (3) years of post-registration clinical experience in Singapore Singapore; current and valid Singapore Nursing Board (SNB) registration; completion of the MOH-approved Continuing Professional Development (CPD) modules within the preceding 24 months; and a clean police clearance certificate issued by the Singapore Police Force. The primary Nurse will be a Registered Nurse (RN) with a Bachelor of Nursing degree, while the supporting Nurse will be an Enrolled Nurse (EN) with a Diploma in Nursing. All Nurse personnel will undergo a pre-assignment health screening and background verification as mandated by HSA guidelines applicable in Singapore Singapore.
All Nurse services outlined in this Quotation Estimate will be delivered within the geographical boundaries of Singapore Singapore, encompassing the Central Region, East Region, West Region, and North Region. The Nurse team will operate from the client's residential address located at 45 Bukit Timah Road. For emergency call-outs, the Nurse will be dispatched to any location within Singapore Singapore within a maximum response time of forty-five (45) minutes. Scheduled visits will be conducted in accordance with the care plan agreed upon at the commencement of the engagement, and any modifications to the Nurse's schedule will require a minimum of 24 hours' prior written notice.
- Payment Schedule: A 30% deposit of the total Quotation Estimate amount (SGD 13,714.38) is due upon acceptance. The remaining 70% shall be invoiced monthly in arrears, payable within fourteen (14) calendar days of invoice issuance.
- Payment Method: Bank transfer to Meridian Care Solutions Pte. Ltd. (DBS Bank, A/C 012-3456-789) or via approved corporate credit card. All payments are in Singapore Dollars (SGD).
- Validity: This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. Prices are subject to revision upon expiry due to fluctuations in Nurse market rates, regulatory fee changes, or supply cost adjustments in Singapore Singapore.
- Termination: Either party may terminate the Nurse engagement with a minimum of thirty (30) days' written notice. In the event of early termination, a pro-rata charge for services rendered up to the termination date will apply.
- Liability & Insurance: Meridian Care Solutions Pte. Ltd. maintains professional indemnity insurance of not less than SGD 2,000,000 per claim covering all Nurse personnel. The client acknowledges that the Nurse acts as an independent professional and that this Quotation Estimate does not constitute a guarantee of specific medical outcomes.
- Confidentiality: All patient health information handled by the Nurse in Singapore Singapore shall be protected in accordance with the Personal Data Protection Act (PDPA) 2012 and the MOH Code of Ethics for Nurses.
- Dispute Resolution: Any disputes arising from this Quotation Estimate shall be resolved through mediation under the Singapore Mediation Centre, and failing that, through the courts of Singapore Singapore.
By signing below, the client acknowledges and accepts all terms, conditions, and cost figures set forth in this Quotation Estimate for Nurse services in Singapore Singapore. This document, once countersigned by both parties, constitutes a binding service agreement for the duration specified herein.
