This Quotation Estimate is prepared in response to the request from St. Mary's Community Health Centre for the provision of qualified Nurse services to be deployed at their facility located in Uganda Kampala. All Nurse personnel engaged under this Quotation Estimate shall hold valid registration with the Uganda Nurses and Midwives Council (UNMC) and possess a minimum of three (3) years of clinical experience in a hospital or community health setting within Uganda Kampala or its surrounding districts.

- This Quotation Estimate is valid for a period of thirty (30) days from the date of issue. Beyond this period, all prices for Nurse services in Uganda Kampala are subject to revision based on market conditions and statutory wage adjustments.
- Payment terms: 50% advance payment upon acceptance of this Quotation Estimate, with the remaining 50% payable in equal monthly installments over the twelve (12) month contract period. Payments shall be made via bank transfer to the account of MediCare Professional Nursing Services Ltd., Bank of Uganda, Kampala Branch.
- All Nurse personnel deployed under this arrangement shall be subject to a standard employment contract compliant with the Employment Act, 2006 of Uganda. The client shall not be directly liable for employment disputes; all HR matters shall be managed by MediCare Professional Nursing Services Ltd.
- The client reserves the right to request the replacement of any Nurse who fails to meet the performance standards outlined in the service-level agreement. Replacement shall be effected within fourteen (14) calendar days at no additional cost to the client.
- This Quotation Estimate does not include the cost of medical equipment, pharmaceuticals, or facility infrastructure. These shall be procured separately by the client at their own expense.
- Any changes to the scope of Nurse services, number of personnel, or deployment locations within Uganda Kampala must be communicated in writing and will be subject to a revised Quotation Estimate before implementation.
- Disputes arising from this Quotation Estimate shall be resolved through amicable negotiation. In the event of failure to resolve, the matter shall be referred to arbitration under the Arbitration Act of Uganda, with the seat of arbitration in Uganda Kampala.
- All Nurse staff shall adhere to the Uganda Nurses and Midwives Council Code of Professional Conduct and the client's internal policies and procedures while on duty at the Uganda Kampala facility.
By signing below, the client acknowledges receipt of this Quotation Estimate for Nurse services in Uganda Kampala and agrees to the terms and conditions stated herein. This signature constitutes formal acceptance of the quoted rates and scope of work.
