1. This Quotation Estimate is valid for thirty (30) days from the date of issue. Upon expiry, rates and availability of the assigned Nurse may be subject to review.

3. A minimum engagement period of three (3) months applies. Early termination prior to the end of this period will incur a cancellation fee equivalent to one month's service fee as detailed in this Quotation Estimate.
4. Any changes to the scope of Nurse services, including additional clinical procedures or extended hours, will require a written amendment to this Quotation Estimate and mutual agreement before implementation.
