This Quotation Estimate covers the provision of a licensed Registered Nurse (RN) with a minimum of five (5) years of acute care experience, specifically trained in geriatric and rehabilitation nursing. The Nurse will be assigned to St. Catherine's Rehabilitation Center, located in the United States Houston area, and will perform the following duties:

• Patient assessment, care planning, and ongoing clinical monitoring on a 24/7 rotational basis.
• Administration of prescribed medications, intravenous therapies, and wound care procedures.
• Coordination with physicians, physical therapists, and social workers within the United States Houston healthcare network.

1. This Quotation Estimate is valid for thirty (30) calendar days from the date of issuance. After the expiration date, all rates and availability of the Nurse position are subject to re-evaluation.
2. All services described herein will be performed in strict accordance with the Texas Occupations Code, Chapter 301 (Nursing Practice Act), and all applicable federal regulations governing healthcare delivery in the United States Houston region.
3. The assigned Nurse must hold an active, unencumbered Texas Registered Nurse license and maintain a minimum of 30 hours of continuing education annually as mandated by the Texas Board of Nursing.
4. Payment terms: Net 30 days from the date of invoice. Invoices will be issued on the first business day of each month for the preceding service period. Late payments will incur a 1.5% monthly interest charge.
5. MedCare Professional Nursing Services, LLC shall not be liable for any claims arising from the direct medical decisions of the attending physicians at St. Catherine's Rehabilitation Center.
6. Either party may terminate this engagement with thirty (30) days' written notice. In the event of early termination, all fees for the current billing cycle shall remain due and payable.
7. This Quotation Estimate does not constitute a binding contract until countersigned by both parties and a formal Service Agreement is executed.
8. All patient data handled by the Nurse will be protected under the Health Insurance Portability and Accountability Act (HIPAA) and the Texas Medical Records Act (Texas Health & Safety Code, Chapter 181).
