This Quotation Estimate outlines the professional services to be delivered by a registered Occupational Therapist based in Australia Sydney, in accordance with the Australian Health Practitioner Regulation Agency (AHPRA) standards and the National Disability Insurance Scheme (NDIS) Practice Standards. The following services are proposed for the client's residential and community-based rehabilitation needs within the greater Sydney metropolitan area.

Note: This Quotation Estimate is based on current fee schedules as of June 2025. If the client is an NDIS participant, the Occupational Therapist services outlined above may be eligible for funding under the NDIS Support and Individualisation category. Our team in Australia Sydney will assist with all necessary documentation for NDIS plan management. Medicare rebates may also apply to certain assessment components. Please confirm your funding pathway prior to commencing services. TERMS AND CONDITIONS

1. This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. After the expiry date, fees may be subject to revision based on current Australian market rates and Occupational Therapist availability in the Sydney region.

2. All services will be delivered by a registered Occupational Therapist holding current AHPRA registration and membership with the Australian Occupational Therapy Association (AOTA). The named therapist, Sarah Mitchell, or an equivalently qualified substitute, will conduct all sessions.

3. Payment is due within fourteen (14) days of invoice issuance. Accepted payment methods include bank transfer (BSB: 062-000, Account: 12345678), credit card, and direct NDIS plan management payment. A 2% late payment surcharge applies to overdue accounts in accordance with Australian commercial practice.

4. Cancellations must be provided with at least forty-eight (48) hours' written notice. Sessions cancelled with less than 48 hours' notice will be charged at 50% of the session fee. No-shows will be charged in full.

5. This Quotation Estimate does not constitute a fixed-price contract. The final invoice may vary by up to 10% depending on the actual scope of work required during the engagement. Any significant variation will be communicated to the client in writing prior to additional work being undertaken.

6. All clinical records and reports generated by the Occupational Therapist are the property of the client and are maintained in accordance with the Privacy Act 1988 (Cth) and Australian Privacy Principles. Records are retained for a minimum of seven years as required by NSW Health regulations.

7. This engagement is governed by the laws of the State of New South Wales, Australia. Any disputes arising from this Quotation Estimate shall be resolved through the NSW Civil and Administrative Tribunal (NCAT) or, where applicable, the NDIS Quality and Safeguards Commission.

8. The Occupational Therapist reserves the right to refer the client to other allied health professionals within our Sydney practice network if additional specialist input is deemed clinically necessary. Such referrals will be communicated to the client in writing.
