This Quotation Estimate for Occupational Therapist services in Brazil Brasília shall be settled in Brazilian Reais (BRL) via bank transfer (TED/PIX) to the account of Clínica Terapêutica Brasília Ltda. Payment is due within fifteen (15) business days from the date of invoice issuance. A 10% non-refundable deposit is required to confirm the engagement and reserve the Occupational Therapist's schedule. In the event of cancellation by the client after the deposit has been paid, the deposit shall be forfeited. Late payments shall incur a monthly interest rate of 1.5% in accordance with Brazilian commercial law (Código Civil, Art. 406).

This Quotation Estimate is valid for thirty (30) calendar days from the issue date stated above. Should the client wish to proceed beyond this validity window, a revised Quotation Estimate reflecting any changes in Occupational Therapist professional fees, material costs, or Brasília operational expenses will be issued. Acceptance of this Quotation Estimate constitutes a binding agreement between both parties for the scope of Occupational Therapist services described herein.

All Occupational Therapist services shall be delivered in compliance with the regulations of the Conselho Federal de Terapia Ocupacional (CFTO) and applicable Brazilian health legislation (Lei nº 6.563/1978 and Decreto nº 93.818/1986).
The Occupational Therapist assigned to this engagement shall maintain full professional liability insurance valid throughout the service period in Brasília, D.F.
Client data and clinical records shall be handled in strict accordance with the Brazilian General Data Protection Law (LGPD — Lei nº 13.709/2018).
Sessions cancelled with less than 24 hours' notice shall be charged at 50% of the session fee.
This Quotation Estimate does not include the cost of assistive devices, orthoses, or specialized materials, which will be quoted separately upon recommendation by the Occupational Therapist.
Any scope changes beyond 10% of the original Quotation Estimate value shall require a written amendment signed by both parties.
Disputes arising from this Quotation Estimate shall be resolved under the jurisdiction of the courts of Brasília, D.F., Brazil.
