Payment for the services outlined in this Quotation Estimate is due within 14 calendar days of the date of service delivery. Accepted payment methods include Interac e-Transfer, major credit cards (Visa, Mastercard, American Express), and certified cheque payable to Coastal Pacific Occupational Therapy Services Inc. A 1.5% monthly late fee will be applied to outstanding balances. The client may apply for coverage through their extended health insurance plan or through the British Columbia Medical Services Plan (MSP) where applicable. The Occupational Therapist will provide itemized receipts for all billing purposes.
Terms and Conditions- This Quotation Estimate is valid for 30 days from the date of issue. After the expiry date, rates may be subject to revision based on current fee schedules established by the College of Occupational Therapists of British Columbia.
- All services will be performed by a registered Occupational Therapist or, where noted, by a supervised Occupational Therapy Assistant under the direct supervision of the lead therapist.
- Home-visit services are limited to the City of Vancouver and the immediate Canada Vancouver metropolitan area. Travel beyond a 25-kilometre radius from the provider's office will incur an additional per-kilometre charge of $0.85 CAD.
- Cancellations or rescheduling of appointments must be made at least 24 hours in advance. Late cancellations or no-shows will be billed at 50% of the session fee.
- The Occupational Therapist reserves the right to modify the treatment plan based on clinical progress. Any additional sessions beyond the quoted package will be billed at the standard rate of $155.00 per 60-minute session plus applicable taxes.
- All client information will be handled in strict compliance with the Personal Information Protection Act (PIPA) of British Columbia and the federal Privacy Act of Canada. Records will be retained for a minimum of 10 years as required by provincial regulation.
- This Quotation Estimate does not constitute a guarantee of specific clinical outcomes. The Occupational Therapist will use best professional judgment and evidence-based practice to achieve the best possible results for the client.
- Disputes arising from this agreement will be governed by the laws of the Province of British Columbia and the federal laws of Canada applicable therein.
