1. This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. After the expiry date, all rates and conditions may be subject to revision based on market adjustments in the Italy Milan healthcare sector.
2. The Occupational Therapist named in this Quotation Estimate, Dr. Elena Marchetti, holds a valid professional license issued by the Regione Lombardia and maintains full professional indemnity insurance with a minimum coverage of € 1,000,000 per claim, as required by Italian legislation (D.Lgs. 206/2007).
3. Payment terms: 60% of the monthly fee shall be invoiced on the 1st of each month, and the remaining 40% on the 15th. Payment shall be made via bank transfer (bonifico bancario) to the IBAN specified on the invoice. Late payments shall incur a penalty of 1.5% per month in accordance with Italian civil code provisions.
4. The Occupational Therapist shall comply with all data protection obligations under the Italian Privacy Code (D.Lgs. 196/2003) and the EU General Data Protection Regulation (GDPR – Regulation 2016/679). 6. Any modifications to the scope of services, frequency of sessions, or additional patient caseload beyond the parameters outlined in this Quotation Estimate shall require a written amendment signed by both parties.
7. The Occupational Therapist reserves the right to refer complex cases to specialist consultants within the Milan healthcare network. Any such additional specialist fees shall be itemized separately and require prior written approval from the client.
8. In the event of force majeure, including but not limited to public health emergencies affecting the Italy Milan region, either party may suspend services without penalty, provided that written notice is given within 48 hours.
