The Occupational Therapist engaged under this Quotation Estimate will provide the following specialized services to patients and residents of the facility in Japan Kyoto:
- Assessment & Evaluation: Comprehensive occupational therapy assessments for patients recovering from stroke, orthopedic injuries, neurological conditions, and age-related functional decline. Each assessment session is conducted by the Occupational Therapist in a dedicated therapy room within the Japan Kyoto facility.
- Individual Therapy Sessions: One-on-one Occupational Therapist sessions focusing on fine motor skill development, adaptive equipment training, daily living activity rehabilitation, and cognitive rehabilitation programs.
- Group Therapy Programs: Supervised group sessions led by the Occupational Therapist, designed to promote social reintegration, handcraft activities, and community-based functional training for up to eight participants per session.
- Home Visit Services: The Occupational Therapist will conduct bi-weekly home visits within the Japan Kyoto city limits to evaluate home environments, recommend adaptive modifications, and provide in-home therapeutic guidance to patients and their caregivers.

- Payment shall be made in quarterly installments of ¥1,037,500 (plus applicable JCT) via bank transfer to the account specified by the Occupational Therapist's practice.
- The first installment is due within 14 calendar days of the effective start date of the service agreement.
- Subsequent installments are due on the 1st of January, April, July, and October of the contract year.
- Late payments shall incur a penalty of 1.5% per month on the outstanding balance.
- This Quotation Estimate is valid for thirty (30) days from the date of issue. Beyond this period, pricing may be subject to revision based on market conditions in Japan Kyoto.
- The Occupational Therapist holds a valid national license issued by the Japan Ministry of Health, Labour and Welfare and maintains professional liability insurance with a minimum coverage of ¥100,000,000.
- Either party may terminate the service agreement with sixty (60) days written notice. In the event of early termination, a pro-rated fee for services rendered will be invoiced.
- All patient records and clinical documentation remain the property of the facility in Japan Kyoto and are subject to the Japanese Act on the Protection of Personal Information (APPI).
- The Occupational Therapist shall comply with all applicable Japanese healthcare regulations, facility protocols, and ethical standards of the Japan Occupational Therapy Association.
- This Quotation Estimate does not constitute a binding contract until both parties have executed a formal Service Agreement incorporating the terms herein.
