- This Quotation Estimate is a formal estimate of costs for Occupational Therapist services and does not constitute a binding contract until accepted in writing by the client and countersigned by our authorized representative.
- All Occupational Therapist sessions must be booked at least 48 hours in advance. Cancellations made less than 24 hours prior to the scheduled session will incur a 50% fee of the applicable session rate.
- The Occupational Therapist reserves the right to modify the treatment plan based on clinical progress and will communicate any changes to the client and referring physician in a timely manner.
- Payment is due within fourteen (14) calendar days from the date of this Quotation Estimate acceptance. A 20% deposit is required to secure the initial assessment appointment at our Malaysia Kuala Lumpur centre.
- Home visit services within the Malaysia Kuala Lumpur metropolitan area (including Cheras, Petaling Jaya, Subang Jaya, and Ampang) are included. Visits outside this radius will attract an additional travel surcharge of RM 2.50 per kilometre.
- All clinical records and Occupational Therapist documentation will be maintained in accordance with the Personal Data Protection Act 2010 (PDPA) of Malaysia and the Malaysian Medical Council's code of ethics.
- This Quotation Estimate does not include the cost of adaptive equipment, orthotics, or assistive devices. A separate itemized quotation will be provided upon the Occupational Therapist's recommendation.
- Any disputes arising from this Quotation Estimate shall be resolved under the laws of Malaysia and subject to the jurisdiction of the courts in Malaysia Kuala Lumpur.
