Payment for the services outlined in this Quotation Estimate is due within 30 calendar days of the invoice date. The Occupational Therapist accepts payment via bank transfer to the practice account held at ING Bank, Amsterdam. In the event that the client holds a Dutch health insurance policy (zorgverzekering) that covers occupational therapy under the General Health Insurance Act (Zvw), the practice will submit claims directly to the insurer. The client is responsible for any applicable deductible (eigen risico) or co-payment (vergoeding) as stipulated by their insurance contract. This Quotation Estimate does not constitute a binding contract until formally accepted in writing by the client.
This Quotation Estimate remains valid for a period of 30 days from the date of issue. Should the client wish to proceed with the proposed Occupational Therapist services in Netherlands Amsterdam, a signed copy of this document must be returned to the practice within the validity period. Any changes to the scope of services, session frequency, or treatment duration will require a revised Quotation Estimate issued by the practice.
- All services will be delivered by a registered Occupational Therapist holding a valid BIG registration with the Dutch Council for Health Care Professions (BIG-register).
- The practice adheres to the Dutch Code of Conduct for Occupational Therapists and the General Terms of the Dutch Healthcare Sector.
- Cancellations must be made at least 48 hours in advance; otherwise, 50% of the session fee will be charged.
- Client data is processed in full compliance with the Dutch Implementation Act GDPR (UAVG) and the European General Data Protection Regulation.
- Liability is limited to the total amount specified in this Quotation Estimate.
- Disputes shall be resolved under the jurisdiction of the competent court in Amsterdam, Netherlands.
- The Occupational Therapist reserves the right to refer the client to a specialist or another discipline if the treatment goals cannot be met within the scope of occupational therapy.
