1. Scope of Occupational Therapist ServicesThe Occupational Therapist programme outlined in this Quotation Estimate encompasses a comprehensive, multi-phase rehabilitation plan designed to restore your ability to perform activities of daily living (ADLs), instrumental activities of daily living (IADLs), and work-related tasks. All sessions will be conducted by a registered Occupational Therapist who is a member of the Nigerian Association of Occupational Therapists (NAOT) and holds a valid practising licence issued by the Council for the Regulation of Occupational Therapy in Nigeria. The programme will be delivered at our fully equipped clinic in Nigeria Abuja, which features a dedicated ADL training kitchen, a sensory integration room, a computerised upper-limb rehabilitation suite, and a home-simulation environment.

The full Occupational Therapist programme described in this Quotation Estimate spans approximately twelve (12) weeks from the date of the initial assessment. Sessions will be scheduled between Monday and Friday, 8:00 AM to 5:00 PM, at our clinic in Nigeria Abuja. The Occupational Therapist will coordinate with your primary physician and any allied health professionals (physiotherapist, speech therapist, psychologist) involved in your care to ensure a cohesive, multidisciplinary approach. Should your recovery trajectory require additional sessions beyond the twelve-week period, a supplementary Quotation Estimate will be issued at no additional administrative charge.
4. Payment Terms- A non-refundable deposit of 30% (NGN 563,730) is due upon acceptance of this Quotation Estimate to secure your programme start date.
- The remaining 70% balance (NGN 1,315,370) shall be payable in two equal instalments: 50% at the midpoint (end of Week 6) and 50% upon completion of the programme (end of Week 12).
- Payment may be made via bank transfer to: Abuja Rehabilitation & Wellness Centre, Access Bank, Account No. 0123456789, Branch: Wuse II, Abuja.
- Corporate and insurance clients may request a pro-forma invoice for reimbursement purposes. We accept claims from major Nigerian health insurers including NHIA-accredited providers, AXA Mansard, Leadway Assurance, and Coronation Health.
- Late payments beyond 14 days of the due date will attract a penalty of 2% per month on the outstanding balance.
- This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. Prices are quoted in Nigerian Naira (NGN) and are subject to revision upon expiry.
- All Occupational Therapist services will be rendered by a qualified, registered practitioner. In the event of the primary therapist's unavailability, a substitute of equivalent seniority will be assigned, and you will be notified at least 24 hours in advance.
- Client cancellations of scheduled sessions must be made no less than 24 hours in advance; otherwise, a 50% session fee will apply.
- The Occupational Therapist will maintain strict confidentiality of all clinical records in accordance with the National Health Act 2014 and the Data Protection Regulation (DPR) 2019 of Nigeria.
- Adaptive equipment supplied under Item 6 becomes the property of the client upon full payment. A one-year warranty covers manufacturing defects only.
- This Quotation Estimate does not constitute a medical diagnosis or a guarantee of specific clinical outcomes. Treatment plans may be adjusted by the Occupational Therapist based on clinical progress.
- Any disputes arising from this Quotation Estimate shall be governed by the laws of the Federal Republic of Nigeria, with jurisdiction in the courts of Abuja, FCT.
