This Quotation Estimate is prepared in response to the formal request submitted by the St. Petersburg Regional Rehabilitation Center for the provision of specialized Occupational Therapist services within the Russia Saint Petersburg metropolitan area. Payment for the services outlined in this Quotation Estimate shall be made in three equal installments: 40% upon contract execution, 30% at the midpoint of the 12-week program (week 6), and 30% upon completion of the final discharge assessment. All payments shall be processed in Russian Rubles (RUB) via bank transfer to the account of NeuroRehab Professional Services LLC, Sberbank, Branch Saint Petersburg, Account No. 40702810000000012345, BIC: 044030643. Invoices will be issued in accordance with Russian Federal Tax Service requirements.
Terms and Conditions- This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. After the expiration date, pricing may be subject to revision based on market conditions in the Russia Saint Petersburg healthcare sector.
- The Occupational Therapist reserves the right to modify the treatment plan based on clinical progress, with written notification to the client at least 48 hours in advance of any schedule changes.
- All services will be conducted at the client's facility located in Russia Saint Petersburg or at the patient's residential address within the city administrative boundaries, as specified in the service agreement.
- Confidentiality of all patient medical records and personal data shall be maintained in strict accordance with Federal Law No. 152-FZ "On Personal Data" of the Russian Federation.
- Cancellation of scheduled sessions must be communicated no less than 24 hours in advance. Sessions cancelled with less than 24 hours' notice will be billed at 50% of the session fee.
- This Quotation Estimate does not constitute a binding contract. A formal service agreement must be executed by both parties prior to the commencement of any Occupational Therapist services.
- Force majeure events, including but not limited to government-mandated closures, pandemics, or natural disasters affecting the Russia Saint Petersburg region, shall suspend service delivery without penalty to either party.
By signing below, the client acknowledges receipt of this Quotation Estimate and agrees to the terms, conditions, and pricing structure outlined for the engagement of the Occupational Therapist in the Russia Saint Petersburg service area.
