The Occupational Therapist assigned to this engagement holds a Bachelor's or Master's degree in Occupational Therapy from an accredited institution, is registered with the Saudi Commission for Health Specialties (SCFHS), and possesses a valid professional license to practice in Saudi Arabia. The Occupational Therapist has a minimum of five (5) years of clinical experience in pediatric, adult, and geriatric rehabilitation, with specific expertise in neurological conditions, orthopedic injuries, and developmental disorders. All practice is conducted in full compliance with the regulations set forth by the Ministry of Health of Saudi Arabia and the applicable standards governing therapeutic services in Riyadh.
5.1 This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. After the expiry date, pricing may be subject to revision based on market conditions in Saudi Arabia Riyadh.
5.2 Payment terms: 50% advance payment upon acceptance of this Quotation Estimate, with the remaining 50% payable in two equal monthly installments over the three-month service period.
5.3 All prices are quoted in Saudi Riyals (SAR) and are inclusive of the Occupational Therapist's professional fees. VAT at the current rate of 15% as mandated by the Zakat, Tax and Customs Authority of Saudi Arabia is applied separately as shown above.
5.4 Cancellation or rescheduling of Occupational Therapist sessions must be communicated at least 24 hours in advance. Sessions cancelled with less than 24 hours' notice will be billed at 50% of the session fee.
5.5 The Occupational Therapist reserves the right to refer patients to other specialists within Saudi Arabia Riyadh if the clinical condition exceeds the scope of occupational therapy services.
5.6 All patient records, assessment reports, and progress documentation generated under this engagement are the property of the client and are maintained in accordance with Saudi data protection and medical confidentiality regulations.
5.7 This Quotation Estimate does not constitute a binding contract until formally accepted in writing by both parties and countersigned by the Occupational Therapist's supervising authority.
Important Note: This Quotation Estimate is prepared specifically for the delivery of Occupational Therapist services in Saudi Arabia Riyadh. Any relocation of services to a different city or region within Saudi Arabia may result in additional travel and logistical charges, which will be communicated prior to commencement.
