This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. Payment is due within fourteen (14) days of invoice issuance. We accept payment via bank transfer, PayNow, or major credit cards. A 50% deposit is required to confirm the engagement of the Occupational Therapist and secure the scheduled appointment slots. The remaining balance shall be payable upon completion of the full treatment programme. All fees are quoted in Singapore Dollars (SGD) and are inclusive of professional consultation but exclusive of any adaptive equipment or assistive devices that may be recommended during the home assessment in Singapore Singapore.

1. This Quotation Estimate is a good-faith estimate of costs and is subject to revision should the scope of services change following the initial assessment by the Occupational Therapist.
2. All services will be conducted in strict compliance with the Personal Data Protection Act (PDPA) of Singapore. Client information will be handled with the utmost confidentiality.
3. The Occupational Therapist engaged under this quotation holds a valid registration with the Singapore Association of Occupational Therapists (SAOT) and carries professional indemnity insurance.
4. Sessions scheduled in the Singapore Singapore area are subject to availability. Rescheduling must be communicated at least 48 hours in advance to avoid a no-show fee of SGD 90.00.
5. This quotation does not constitute a medical diagnosis or a guarantee of specific therapeutic outcomes. Treatment progress will be monitored and documented at each review session.
6. Any additional services beyond the scope outlined herein will be subject to a separate written amendment to this Quotation Estimate, signed by both parties.
7. Disputes arising from this engagement shall be resolved in accordance with the laws of the Republic of Singapore and under the jurisdiction of the Singapore courts.
